分化型甲状腺癌是颈部最常见的恶性肿瘤,也是甲状腺癌最主要的类型。甲状腺腺叶切除术或甲状腺全切术是原发灶的主要治疗方法。分化型甲状腺癌早期就存在颈淋巴结转移,通常认为颈淋巴结转移第一站是颈中央区淋巴结,被称为“前哨淋巴结”,所以对于颈中央区颈淋巴结清扫就显得尤为重要。cN1患者需要行颈中央区淋巴结清扫术,但是对于cN0患者是否需要进行预防性颈中央区淋巴结清扫术仍有很大的争议。本文旨在对预防性颈中央区淋巴结清扫术运用于分化型甲状腺癌的相关共识与争论进行讨论。
Differentiated thyroid carcinoma is the most common cervical malignancy,also the most important main type of thyroid cancer.Thyroid lobectomy or total thyroidectomy is the main method for the treatment of primary tumor.Early metastasis to the central cervical lymph node usually occurs in differentiated thyroid carcinoma.Central cervical lymph node is generally considered the first station,called the “sentinel lymph node”,so the central cervical lymph node dissection is particularly important in differentiated thyroid carcinoma .Patients with cN1 require the central cervical lymphadenectomy,but for patients with cN0,the need of prophylactic central neck lymphadenectomy remains controversial.This article aims to discuss the consensus and the controversy on Prophylactic Central Lymph Node Dissection applied in differentiated thyroid cancer.
