目的 探讨椎间孔镜TESSYS（transforaminal endoscopic spine system）技术与传统后路开窗减压术两种术式治疗糖尿病患者伴发腰椎间盘突出症的疗效。方法 回顾分析糖尿病患者伴发腰椎间盘突出症35例，分别采用椎间孔镜TESSYS技术与传统后路开窗减压术治疗，分析两种手术方式下患者术前、术后1月、6月腿痛视觉疼痛模拟评分法（visual analogue scale, VAS）以及Oswestry功能障碍指数（Oswestry disability index, ODI）。结果 椎间孔镜TESSYS技术与后路开窗减压术治疗糖尿病患者腰椎间盘突出症，所有患者术后均感腿痛症状明显缓解。随访两组患者腿痛视觉模拟评分（VAS）术前分别为7.7±1.6、7.9±1.5，术后1月为3.5±0.9、3.3±1.4，术后6个月复查为2.9±0.7、2.7±1.5，两组间差异无统计学意义。两组患者腿痛Oswestry功能障碍指数（ODI）术前分别为65.3±15.1、71.5±14.6，术后1月为16.8±7.9、17.1±8.5，术后6个月复查为15.8±5.6、15.5±6.3，两组间差异无统计学意义。 结论 椎间孔镜TESSYS技术与后路开窗减压术治疗糖尿病患者腰椎间盘突出症都可以达到满意效果。

Objectives To evaluated the effect of transforaminal endoscopic spine system (TESSYS) and open lumbar discectomy for treatment of diabetes patient lumbar disc protrusion. Methods 35 diabetes patient with lumbar disc protrusion were studied. TESSYS and open lumbar discectomy for treatment of diabetes patient lumbar disc protrusion. Between before the operation and after the operation, VAS、ODI for the leg pain were analysis. Results After discectomy, 35 patients leg pains were relieved significantly. Two groups of patients with leg pain visual analogue scale (VAS) before operation were 7.7±1.6,7.9±1.5. After one month 3.5±0.9,3.3±1.4 were recording, 6 months after the treatment was 2.9±0.7,2.7±1.5. There was no significant difference between the two groups(p>0.05). Two groups of patients with leg pain and Oswestry disability index (ODI) before operation were 65.3±15.1,71.5±14.6. After one month, there were 16.8±7.9,17.1±8.5, 6 months after the treatment was 15.8±5.6,15.5±6.3.There was no significant difference between the two groups(p>0.05). Conclusion The TESSYS and open lumbar discectomy in the treatment of diabetes patients with lumbar disc protrusion can achieve satisfactory effect.
