【摘要】目的:分析探讨在快通道心脏麻醉中使用瑞芬太尼的临床效果。方法:选取我院2014年5月至2015年5月收治的120例需要进行心脏病手术的患者进行研究,将所有患者随机分为对照组60例和试验组60例。在手术的过程中,对照组的患者采用的麻醉药物为芬太尼与丙泊酚联合,试验组的患者采用瑞芬太尼与丙泊酚联合进行治疗,通过对两组患者的血流动力学、ICU观察时间、拔管时间以及住院时间进行比较,对两组患者的临床疗效进行比较。结果:两组患者经过麻醉后的MAP和HR明显低于麻醉诱导前,对照组的降低程度明显大于试验组的降低程度,试验组患者ICU观察的时间、拔管的时间、住院的时长明显短于对照组患者ICU观察的时间、拔管的时间以及住院的时长,差异具有统计学意义(p<0.05)。结论:在快通道心脏麻醉的过程中使用瑞芬太尼有着显著的临床疗效,血流动力学更加稳定的同时能够缩短拔管和在ICU观察的时间,减短了患者住院的时长,值得在临床上大力推广使用。
Abstract Objective: To investigate the clinic effect of remifentanil on fast tracking cardiac anesthesia. Methods: 120 cases in our hospital from May 2014 to May 2015 were selected as study objects. They were divided into control group and observation group, 60 cases in each group. The control group was given fentanyl combined with propofol. The observation group was given remifentanil and propofol. Compared the hemodynamics, ICU observation time, extubation time, hospital stays of the two groups and clinic effect of the two groups. Results: MAP and HR post-anesthesia of the two groups was obviously lower than before-anesthesia. Decreased degree of control group was obviously greater than observation group. ICU observation time, extubation time, hospital stays of observation group was obviously shorter than control group, the difference was statistically significant (p<0.05). Conclusion: Application of remifentanil on fast tracking cardiac anesthesia has significant clinic effect. Hemodynamics is more stable and can shorten the ICU observation time, extubation time and hospital stay. It is worthy of wide application.
