目的 通过对14例晚期胰腺癌患者瘤体内注射131I-chTNT(131I标记的肿瘤细胞核单克隆抗体)治疗胰腺癌的护理观察,总结131I-chTNT单克隆抗体放射免疫治疗胰腺癌的护理要点。方法 所有胰腺癌患者(n=14)均采用计算机断层扫面引导下瘤体内直接注射131I-chTNT注射液(50 mCi),并给于全面细致的护理,包括术前健康教育及术后并发症护理与观察。 结果 14例患者在科学、系统、精心的治疗护理下,手术全部顺利完成,成功率100%,随访完全缓解(CR) 1例(7%),部分缓解(PR) 4例(29%),稳定(SD)7例(50%),进展(PD)2例(14%),有效率CR+PR 36%,未发生严重并发症。结论 足够的剂量131I-chTNT单克隆抗体瘤体内注射对于晚期胰腺癌的治疗效果显著,是一种安全有效的新方法,围手术期加强心理护理,生命指征监测,并发症的预防及护理,是确保手术成功及防止相关并发症的有力保障。
Objective To summarize the nursing key points of radioimmunotherapy of pancreatic cancer by observing the nursing care in 14 patients with advanced pancreatic cancer receiving injection of 131I-chTNT (iodine-131-lablbed tumor necrosis therapy chimeric antibody).Methods All pancreatic cancer patients (n=14) were treated with computed tomography(CT) guided 131I-chTNT intratumoral injection, and received comprehensive nursing including preoperative health education and postoperative complication nursing care.Results All patients were successfully treated with 131I-chTNT intratumoral injection with scientific, systematic, and meticulous nursing.The success rate was 100%.Specifically, 1 case had a complete response (CR) (7%), 4 cases achieved partial response (PR) (29%), 7 cases had stable conditions (50%), and 4 cases progressed (14%).The effective rate was 36%.Conclusions These promising Results demonstrate that sufficient doses of 131I-chTNT can be safely delivered to tumors to cause significant therapeutic effects on advanced pancreatic cancer.Perioperative psychological nursing, monitoring of vital signs, prevention and nursing of complications are a powerful guarantee for successful operation and prevention of complications.
