紫草素-季铵盐壳聚糖水凝胶复合物促进糖尿病小腿创面愈合的临床研究

Clinical study on promotion of diabetic lower leg wound healing by shikonin-quaternized chitosan hydrogel composite

摘要: 目的 探讨紫草素(SK)溶液、紫草素-季铵盐壳聚糖复合水凝胶(SK-QPP)治疗糖尿病小腿创面的效果及安全性。方法 选取2025年1—12月收治的60例糖尿病所致小腿创面患者,随机分为SK组、SK-QPP组及对照组,每组各20例。对照组行常规处理,另两组清创后分别外敷SK溶液、SK-QPP,比较三组创面愈合率、有效率、愈合时间及不良反应。结果 治疗1、2、4周,愈合率SK-QPP组>SK组>对照组(P<0.05);创面愈合时间SK-QPP组(16.24±2.87) d<SK组(19.68±3.12) d<对照组(28.53±4.21) d(P<0.05);SK-QPP组治疗有效率最高(95.0%),>SK组(85.0%)>对照组(65.0%)(P<0.05);三组间不良反应发生率差异无统计学意义(P>0.05)。结论 SK溶液及SK-QPP均能提升糖尿病小腿创面愈合效果、缩短疗程且安全性好,且SK-QPP疗效更优,值得推广。

Abstract: Objective To investigate the efficacy and safety of shikonin (SK) solution and shikonin-quaternized chitosan hydrogel composite (SK-QPP) in the treatment of diabetic lower leg wounds. Methods Sixty patients with diabetic lower leg wounds admitted to our hospital from January to December 2025 were selected and randomly divided into an SK group, an SK-QPP group, and a control group, with 20 cases in each group. The control group received routine treatment, while the other two groups were treated with topical SK solution and SK-QPP after debridement, respectively. Wound healing rate, effective rate, healing time, and adverse reactions were compared among the three groups. Results At 1, 2 and 4 weeks of treatment, the wound healing rate rank was SK-QPP group > SK group > control group (P<0.05). The wound healing time was (16.24±2.87) d in the SK-QPP group, (19.68±3.12) d in the SK group, and (28.53±4.21) d in the control group (P<0.05). The effective rate was the highest in the SK-QPP group (95.0%), followed by the SK group (85.0%) and the control group (65.0%) (P<0.05). There were no significant differences in adverse reactions among the three groups (P>0.05). Conclusion Both SK solution and SK-QPP can improve the healing effect of diabetic lower leg wounds, shorten the treatment course, and show good safety, and SK-QPP has better efficacy and is worthy of clinical promotion.
