外侧入路倒置Philos钢板及内侧螺钉行胫距跟关节融合的疗效分析

摘要: 目的 探讨外侧入路倒置Philos钢板结合内侧空心螺钉行胫距跟关节融合的临床疗效。方法 2012年6月至2014年12月采用腓骨外侧入路倒置Philos钢板及内侧空心螺钉行胫距跟关节融术治疗胫距跟关节病变患者12例,术后观察创面愈合情况,并于术后1月、3月、6月及12月观察患踝疼痛、并发症、影像学胫距跟关节融合及内固定情况,对踝关节功能评估采用美国足踝外科协会(AOFAS)踝-后足评分标准。结果 术后均获随访,平均随访时间18.6月(12-36月)。术后切口愈合良好,无感染及坏死;1例术后出现足前外侧麻木,对症处理3个月后症状消失;术后3月随访患踝无明显疼痛,X片及踝关节CT显示踝关节基本融合、内固定无松动断裂;术后12月患踝均无疼痛,影像学显示患踝均已融合,内固定位置良好,AOFAS踝-后足评分平均为77.5分。结论 应用外侧入路倒置Philos钢板及内侧空心螺钉行胫距跟关节融术临床疗效良好,可有效缓解疼痛,矫正畸形并改善功能,是治疗严重胫距跟关节病变的有效方法。

Abstract: Objective: To evaluate clinical efficacy of the lateral incision for tibial talocalcaneal Arthrodesis by using inverted Philos Plate and Medial Hollow screw. Methods: In between June 2012 to December 2014, lateral approach incision of ankle inversion Philos plate and the medial tibial subtalar joint with cannulated screws for the treatment of 12 cases of ankle joint lesion, post-operative follow up after 1 month and 3,6,12 months, for observation of wound healing, pain, subtalar Joint Fusion, fixation and ankle function. Results Patients were followed up after a mean follow-up time of 18.6 months. After 2 weeks of operation incision healed properly, no infection and the tissue necrosis; lateral side numb ness observed in one case, which disappeared after 3 months. No one complain about painful movement of ankle joint, on CT examination good fusion of ankle joint, and none of the internal fixators loose after 3 months of operation. After 12 months of operation, no one complained about painful movement ankle joint, good fusion and no any migration of the internal fixators. Ankle and lateral foot score average was77.5 according to AOFAS score. Conclusion: clinical efficacy of the lateral incision for tibial talocalcaneal Arthrodesis by using inverted Philos Plate and Medial Hollow screw has excellent clinical AOFAS score, which can meet the livelihood desire for relieving ankle joint pain. As well this is also effective on severe deformity correction of ankle joint.
