非离断式Roux-en-Y吻合在腹腔镜远端胃癌根治消化道重建中的应用效果分析

摘要： 目的 探讨非离断式Roux-en-Y吻合在腹腔镜远端胃癌根治消化道重建手术中的临床效果。方法 回顾性分析2014年1月—2018年12月接受腹腔镜远端胃癌根治术的80例胃癌患者,按消化道重建方式分为非离断式Roux-en-Y吻合组（观察组）40例和Billroth Ⅱ吻合组（对照组）40例,比较两组的手术指标（手术时间、术中出血量）和术后的排气时间、并发症（反流性胃炎、十二指肠残端瘘、肠梗阻、胃排空障碍）及住院时间。结果 观察组术中出血量、手术时间小于对照组（均P<0.01）;两组术后反流性胃炎发生率小于对照（P<0.01）、住院时间无差异）。结论 非离断Roux-en-Y吻合在腹腔镜远端胃癌根治消化道重建中应用近期效果理想、安全。

Abstract: Objective To evaluate the clinical effect of non-dissected Roux-en-Y anastomosis on laparoscopic radical gastrectomy for distal gastric cancer and digestive tract reconstruction. Methods A total of 80 patients with gastric cancer who underwent laparoscopic radical gastrectomy from January 2014 to December 2018 were divided into non-dissected Roux-en-Y anastomosis group (observation group, 40 cases) and Billroth II anastomosis group (control group, 40 cases) according to digestive tract reconstruction methods. The surgical indicators, postoperative exsufflation time, complication rate, and hospitalization stay were compared between the two groups. Results The intraoperative bleeding amount and operation duration of the observation group were more than those of the control group (P<0.01), but no significant differences in complication rate and hospitalization time were observed between the two groups. Conclusion Non-dissected Roux-en-Y anastomosis is effective and safe in laparoscopic radical gastrectomy for distal gastric cancer and digestive tract reconstruction.
