目的 探讨颅脑创伤后顽固性高颅压采取大骨瓣减压术治疗的临床疗效。方法 选取86例于2014年4月—2017年4月期间收治的颅脑外伤后顽固性高颅压患者,随机分为对照组(n=43,予以常规骨瓣开颅减压术治疗)与观察组(n=43,予以大骨瓣减压术治疗),观察患者疗效。结果 观察组患者恢复良好率优于对照组(P<0.05);术后3个月时,观察组患者NIHSS评分低于对照组(P<0.05);观察组患者并发症发生率低于对照组(P<0.05)。结论 颅脑创伤后顽固性高颅压采取大骨瓣开颅减压术治疗,可提高治疗疗效,有效改善术后神经功能,并发症少,具有推广价值。

Objective To explore the clinical effect of large decompressive craniectomy on craniocerebral trauma complicated with intractable high cranial pressure.Methods A total of 86 craniocerebral trauma patients complicated with intractable high cranial pressure treated from April 2014 to April 2017 were selected and randomly divided into two groups,with 43 cases in each group.The control group adopted conventional bone flap craniotomy.The observation group adopted large decompressive craniectomy.The clinical effects in the two groups were observed.Results The recovery rate of the observation group was significantly higher than that of the control group (P＜0.05).After 3 months of surgery,the NIHSS score of the observation group was significantly lower than that of the control group (P＜0.05).The incidence rate of complications of the observation group was significantly lower than that of the control group (P＜0.05).Conclusion For craniocerebral trauma patients complicated with intractable high cranial pressure,large decompressive craniectomy can improve the clinical effect and neurological function,and reduce the incidence rate of complications.It is worthy of clinical promotion.
