第二眼超声对MRI额外发现的乳腺BI-RADS 4或5类病变的诊断价值

Value of second-look ultrasound in diagnosis of BI-RADS category 4 or 5 breast lesions detected additionally by MRI

摘要: 目的 探讨第二眼超声对磁共振成像(MRI)额外发现的乳腺影像报告和数据系统(BI-RADS) 4或5类乳腺病变的诊断价值。方法 回顾性研究了MRI额外检查到BI-RADS 4或5类乳腺病变(89个)的第二眼超声影像学特征及BI-RADS分类。结果 MRI检查额外发现的89个BI-RADS 4或5类乳腺病变,经第二眼超声检查可匹配病变79个,检出率为88.8%,其中恶性病变22个(27.8%)。超声可匹配乳腺病变中,边界和血流分布两项超声特征在良、恶性组之间差异具有统计学意义(P<0.05)。第二眼超声再评估的受试者工作特征(ROC)曲线下面积为0.800,诊断乳腺癌的准确率较高。结论 第二眼超声能有效检出MRI额外发现的乳腺BI-RADS 4或5类病变且具有较高的诊断价值。

Abstract: Objective To investigate the diagnostic value of second-look ultrasound (SLU) for additional breast lesions of Breast Imaging Reporting and Data System (BI-RADS) category 4 or 5 detected by MRI. Methods A total of 89 suspicious lesions of BI-RADS category 4 or 5 detected additionally by magnetic resonance imaging (MRI) were retrospectively analyzed by SLU for ultrasound imaging features and BI-RADS category. Results Of the 89 additional lesions detected by MRI, 79 lesions were detected by SLU, and the detection rate was 88.8%, including 22 (27.8%) malignant lesions. There were statistically significant differences between benign and malignant groups in the boundary and blood flow distribution of mass (P<0.05). The area under the receiver operating characteristic (ROC) curve of SLU re-evaluation was 0.800, indicating a high accuracy in diagnosing breast cancer. Conclusion SLU can effectively detect additional BI-RADS category 4 or 5 lesions by MRI and has a high diagnostic value.
