Abstract: Xanthogranulomatous cholecystitis (XGC), a rare and special type of cholecystitis, is identified as benign lesions but tends to invade and destroy the surrounding tissues and organs, and its clinical characteristics are not specific. There is no consensus on its diagnosis and treatment, especially the differential diagnosis with gallbladder cancer (GBC). Conflicts and controversies have always run through the choice of preoperative diagnosis and surgical treatment of XGC. This article mainly reviewed the incidence, clinical characteristics, and the difficulties in diagnosis and treatment of XGC.

Key words: Xanthogranulomatous cholecystitis, Gallbladder carcinoma, Differential diagnosis, Clinical dilemma
