双镜联合经胆囊管胆总管探查治疗肝外胆管结石疗效观察

摘要： 目的 探索双镜联合经胆囊管胆总管探查、切开治疗肝外胆管结石的临床疗效。方法 以2017年12月1日—2019年1月30日期间收治的40例肝外胆管结石患者为研究对象,随机分为观察组(双镜联合经胆囊管胆总管探查)和对照组(腹腔镜下胆总管切开探查)各20例,对比观察2组的疗效和安全性。结果 观察组并发症发生率低于对照组,手术时间、术中出血量少于对照组,胃肠道功能恢复时间、腹腔引流时间短于对照组,丙氨酸转氨酶、碱性磷酸酶、谷氨酰转肽酶、胆碱酯酶优于对照组(P＜0.05)。结论 肝外胆管结石实施双镜联合经胆囊管胆总管探查取石效果显著。

Abstract: Objective To explore the clinical efficacy of double-mirror combined with cystic duct and common bile duct exploration and incision for extrahepatic bile duct stones.Methods Forty patients with extrahepatic bile duct stones from December 1, 2017 to January 30, 2019 were randomly divided into observation group (double-mirror combined with cystic duct and common bile duct exploration) and control group (laparoscopic common bile duct exploration).The clinical efficacy and safety between the two groups were compared.Results The observation group showed lower incidence rate of complications, operation time, intraoperative blood loss, recovery time of gastrointestinal function, and drainage time of abdominal cavity than the control group (P<0.05).The alanine aminotransferase, alkaline phosphatasey, glutamyl transpeptidase, and cholinesterase of the observation group were superior to those of the control group (P<0.05).Conclusion Double-mirror combined with cystic duct and common bile duct exploration is effective and safe for extrahepatic bile duct stones.
