伤科骨愈汤辅助交锁髓内钉内固定治疗胫腓骨骨折患者临床疗效研究

Effect of Shangke Guyu Decoction in patients with tibiofibular fracture treated with interlocking intramedullary nail internal fixation

摘要: 目的 探究伤科骨愈汤辅助交锁髓内钉内固定治疗胫腓骨骨折患者康复进程、骨代谢及血清炎性因子水平等临床疗效的影响。方法 选取2020年1月—2022年1月莆田福德医院骨科收治的80例胫腓骨骨折患者作为研究对象,采用信封法将所有患者分为手术组(40例)与中医辅助组(40例)。手术组行交锁髓内钉内固定治疗,中医辅助组则在交锁髓内钉内固定的基础上采用伤科骨愈汤进行辅助治疗。比较两组患者的康复效果、骨代谢及血清炎性因子水平。结果 中医辅助组的胫腓骨骨折愈合时间、术后下床活动时间、住院时间均明显短于手术组(P<0.05);中医辅助组的血清碱性磷酸酶、骨钙素、Ⅰ型前胶原羧基末端前肽水平均高于手术组(P<0.05);中医辅助组的肿瘤坏死因子α、白介素1、白介素6水平均低于手术组(P<0.05)。结论 伤科骨愈汤辅助交锁髓内钉内固定治疗胫腓骨骨折能够有效缩短患者的康复进程,改善骨代谢水平,同时减少机体内的炎性因子。

Abstract: Objective To explore the effects of Shangke Guyu Decoction on rehabilitation, bone metabolism, and serum inflammatory factors in patients with tibiofibular fracture treated with interlocking intramedullary nail internal fixation. Methods From January 2020 to January 2022, 80 patients with tibiofibular fracture admitted to the Department of Orthopedics of the Putian Fude Hospital University were selected as the study subjects. All patients were divided into an operation group (40 cases) and an auxiliary group of traditional Chinese medicine (TMC) (40 cases) by envelope method. The operation group was treated with interlocking intramedullary nail internal fixation, while the TMC auxiliary group was treated with Shangke Guyu Decoction additionally. The rehabilitation effect, bone metabolism, and serum inflammatory factor levels of the two groups were compared. Results The healing time of tibia and fibula fracture, the time of getting out of bed after operation, and the time of hospitalization in the TCM auxiliary group were significantly shorter than those in the operation group (P<0.05). The levels of serum alkaline phosphatase, osteocalcin, and C-terminal peptide of type Ⅰ procollagen in the TCM auxiliary group were higher than those in the operation group (P<0.05). The levels of tumor necrosis factor α, IL-1, and IL-6 in the TCM auxiliary group were lower than those in the operation group (P<0.05). Conclusion The treatment of tibia and fibula fracture with Shangke Guyu Decoction and interlocking intramedullary nailing can effectively fasten the rehabilitation process of patients, improve bone metabolism, and reduce inflammatory factors.
