    Thermal ablation in treatment of benign and malignant thyroid tumors: Current status and future perspectives

    In recent years the application of thermal ablation (TA) in the treatment of benign and malignant thyroid tumors has drawn wide attention. Many studies have shown that compared with traditional surgery, TA is a minimally invasive and cosmetic surgery. In particular, the safety and effectiveness of TA in the treatment of benign thyroid tumors have been generally proven, but are still controversial in the treatment of papillary thyroid microcarcinoma. This article reviewed related literature, and focused on the principles of different thermal ablation techniques, indications and contraindications for the treatment of benign and malignant thyroid tumors, the safety and effectiveness of TA, and the advantages and disadvantages of TA compared with traditional surgery.

    With the continuation of the COVID-19 epidemic, China attaches more and more importance to responding to public health emergencies, and epidemic prevention and control as well as standardized and effective disinfection methods have become an immediate need for hospitals. Thanks to the development of science and technology, intelligent disinfection robots have become one of the options for hospital disinfection operations. This paper explores the feasibility of intelligent disinfection robots as a new approach to hospital disinfection by comparing the methods of manual disinfection versus intelligent robot disinfection, and using scenario simulation experiments to demonstrate the effect of robot disinfection.

    Observation of clinical effect of elastic thread ligation on stage Ⅲ mixed hemorrhoids

    Objective To study the clinical effect of elastic thread ligation on stage III mixed hemorrhoids. Methods Eighty patients with stage III mixed hemorrhoids were divided into a study group (n=40) and a control group (n=40) randomly. The study group was treated with elastic thread ligation, while the control group was treated with tissue-selecting therapystapler (TST). Operation time, blood loss volume, postoperative complications, healing time, recurrence rate, and other indicators were observed. Results In the study group: operation time, (18.0±3.5) min; intraoperative blood loss volume, (5.0±1.0) mL; postoperative hemorrhage, 0 case (0); anal pendant expansion, 2.5% (1/40); healing time, (18.2±2.5) d. In the control group: operation time, (25.0±7.0) min; intraopereative blood loss volume, (25.0±6.5) mL; postoperative hemorrhage, 5% (2/40); anal pendant expansion, 15% (6/40); healing time, (23.8±3.9) d. The study group showed better performance in terms of operation time, blood loss volume, postoperative complications, and healing time than the control group (P<0.05). Conclusion Elastic thread ligation treatment has advantages in reduced operation time, bleeding amount, complications, and healing time, so it is worthy of clinical promotion.

    Objective To summarize the experience of clinical research information management platform based on integrated digital thinking mode. Methods The links of project approval, start-up preparation, project implementation, quality supervision, and conclusion were connected, and then the informatization and standardization of the whole process were realized in combination with the guidance of clinical research at all levels. Results The construction of the clinical research information management platform provided high-quality data assets and helped hospital digital transformation. Conclusion The digital clinical research management platform has greatly standardized the process of clinical research in the hospital, realized efficient management of clinical research, and promoted the medical quality, medical scientific research exploration, and the construction and development of clinical disciplines.

    Safety and efficacy of radiofrequency ablation for hyperthyroidism

    Objective To explore the safety and efficacy of ultrasound-guided radiofrequency ablation (RFA) in the treatment of hyperthyroidism. Methods Twenty-seven patients with hyperthyroidism from The Second Affiliated Hospital of Xiamen Medical College in the period of July 2018 to June 2019 were treated with ultrasound-guided RFA. Thyroid volume, total triiodized thyroid hormone (TT3), free triiodized thyroid hormone (FT3), total tetriodized thyroid hormone (TT4), free tetriodized thyroid hormone (FT4), thyroid stimulating hormone (TSH), clinical symptoms, and complications were compared before and after RFA. Results The clinical symptoms of patients were significantly improved after surgery (P<0.05). The thyroid volumes were significantly reduced 3, 6, 12 months after surgery (P<0.05). Serum TT3, TT4, FT3, and FT4 levels decreased significantly and TSH increased significantly after surgery (P<0.05). The hoarseness rate reached 3.7% and tachycardia rate reached 11.1%, which all recovered after treatment without recurrence and no other complications occurred. Conclusion Ultrasound-guided percutaneous RFA is a safe and effective method for hyperthyroidism, which can improve the symptoms significantly with few complications.

    Osteogenesis imperfecta, also known as brittle bone-blue sclera-deafness syndrome, is a congenital genetic disease caused by mesenchymal tissue hypoplasia and collagen formation disorder. The main clinical diagnostic criteria are: (1) blue sclera; (2) osteoporosis and increased bone fragility; (3) dentin hypoplasia; (4) early hearing loss. A diagnosis is made when two of the above criteria are met. At present, there is no effective treatment for the pathogenic gene mutation of osteogenesis imperfecta. The existing treatment is only symptomatic treatment, aiming to increase bone density, reduce fracture rate, correct bone deformity, and improve the quality of life of the patients.

    Application of PDCA to improve standard rate of emergency deployment equipment use in emergency rescue rooms

    Objective To explore the application of PDCA in improving the standard rate of emergency deployment equipment use in emergency rescue rooms. Methods PDCA cycle was applied to the management of emergency deployment equipment in emergency rescue rooms. A project quality improvement group was established based on the focus of current problems and cause analysis, and carried out continuous quality improvement according to the PDCA process of planning, doing, checking, and acting. Results The standard rate of emergency deployment equipment use in emergency rescue rooms increased from 76.25% before implementation to 95.00% after implementation, indicating that the nurses in rescue rooms improved their standard operation ability regarding emergency deployment equipment. Conclusion The application of PDCA can effectively improve the standard rate of emergency deployment equipment use of nurses in emergency rescue rooms, and ensure the safety of clinical nursing.

    Rotator cuff tear is a common shoulder disease, especially among athletes and middle-aged and elderly individuals. With the accelerating trend of aging population, the number of patients with rotator cuff tears is increasing. For massive rotator cuff tears, there is a high risk of re-tear after surgery and poor prognosis, which poses a significant clinical challenge. In recent years, surgical techniques for massive rotator cuff tears have continued to innovate, including superior capsule reconstruction, subacromial balloon spacer implantation, tendon transfer, and reverse shoulder arthroplasty. This article summarized the biomechanical effects of massive rotator cuff tears on the shoulder joint and focused on the latest treatment advancements, aiming to provide new reference for clinical treatment.

    Objective To investigate the clinical effect of local infiltration anesthesia combined with preperitoneal modified Kugel hernia repair on inguinal hernia in elderly female patients. Methods The clinical data of elderly female patients over 60 years old admitted to our hospital from September 2017 to September 2020 were retrospectively analyzed, and patients receiving local anesthesia +modified Kugel repair were selected as study subjects. Postoperative pain, recovery time of non-restricted activity, recurrence of hernia, and complications during follow-up were observed. Results Nineteen patients who met the criteria were followed up, ranging in age from 60 to 81 years old, with an average age of (64.5±3.3) years. Superficial infection of postoperative incision occurred in 1 patient and labia edema occurred in 1 patient. Overall, postoperative pain response was mild and satisfaction was good. The average follow-up time was (13.0±3.0) months. No hernia recurrence was observed, and no chronic pain was found. Conclusion Local infiltration anesthesia combined with preperitoneal modified Kugel hernia repair is an ideal surgical procedure for elderly female inguinal hernia patients who cannot tolerate general anesthesia.

Recently, new targeted drugs, immuno-targeted combination therapy, HIF-2α inhibitors, and probiotic combination therapy provide more possible options for the treatment. In this review, we gave an overview of the advances in systemic therapy of advanced renal cell carcinoma, aiming to help make clinical decisions.

    Objective To make full use of large-scale medical equipment, improve the public welfare and medical efficiency of hospitals, and effectively supervise the use and efficiency of large-scale equipment. Methods By establishing a new large-scale equipment cloud management platform based on the Internet of Things technology to monitor equipment usage and efficiency in real time, where administrators directly conduct a comprehensive analysis of equipment efficiency on the cloud platform to maximize the equipment use efficiency. Results Through the management of the use of large-scale medical equipment through the Internet of Things cloud platform, the workload, efficiency, and patient experience of the equipment were intuitively analyzed, which significantly improved the hospital's supervision of large-scale equipment, elevated the efficiency of equipment use, and reduced the waiting time of patients. Conclusion The large-scale equipment cloud management platform supported by the Internet of Things technology has effectively improved hospital medical efficiency, reasonably triaged patients, reduced the waiting time for medical treatment, implemented social welfare, and laid a solid foundation for future informatization development of the hospital.

    The disposal of medical supplies plays a very important role in public health emergency logistic support. As one of the important parts in epidemic prevention and control, the disposal of medical materials after the closure of large shelter hospitals is crucial for the full utilization of medical equipment and medical consumables, the proper placement of state-owned assets, and the conservation of medical resources. This paper summarized the practical experience in the disposal and management of medical materials after the closure of the Fangcang Shelter Hospital of the National Convention and Exhibition Center (Shanghai), focused on the principle of "overall allocation, full utilization, step-by-step implementation, and combination of use and storage", and formed a material disposal management system through the establishment of material disposal organization structure, division of responsibilities, and operation mechanism, in order to provide reference for further improving the management system of medical material disposal in public health events.

    Objective To investigate the clinical value of diquafosol sodium combined with sodium hyaluronate for patients with dry eye after cataract surgery. Methods In this study, 236 patients with dry eye after cataract surgery, who were treated with diquafosol sodium and sodium hyaluronate in our hospital from January 2022 to January 2023, were selected as the combined group, at the same time, 82 patients with dry eye after cataract surgery treated with sodium hyaluronate alone were selected as the single therapy group, and the therapeutic effects of the two groups were compared and analyzed. Results The total effective rate of the combined group was significantly higher than that of the single therapy group (P<0.05). There was no significant difference in SPEED score, Fluorescent Test (FL), Tear Breakup Time (BUT), and Schirner Test (Slt) between the two groups before treatment (P>0.05). After treatment, the SPEED score, FL, BUT, and Slt in both groups were significantly improved (P<0.05), and the scores of SPEED, FL, BUT, and Slt in the combined group were significantly better than those in the single therapy group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Compared with sodium hyaluronate alone, the combination of sodium hyaluronate and diquafosol sodium can significantly improve the therapeutic effect on dry eye after cataract surgery.

    Comparative study of clinical outcomes of different surgical approaches for cardiac cancer

    Objective To compare the clinical outcomes of patients with cardiac cancer undergoing radical resection by laparotomy or thoracotomy. Methods A total of 198 patients with cardiac cancer were selected from January 2014 to December 2017. Among them, 108 patients in the study group received trans-abdominal cancer resection and 90 patients in the control group received trans-thoracic cancer resection. General clinical characteristics, surgery time, intraoperative and postoperative hemorrhage volumes, postoperative complications, lymph nodes resection rate, positive surgical margins, tumor node metastasis (TNM) stages, hospital mortality rate, hospital stay, and 5-year survival were compared between the two groups. Results No significant difference was found in general clinical characteristics (P>0.05). Compared with the control group, the surgery time, intraoperative and postoperative hemorrhage volumes, postoperative complication rate, lymph nodes resection rate, and hospital stay were superior in the study group (P<0.05). No difference in positive surgical margin rate was found between the two groups, both with complete resection. There was no significant difference in 5-year survival rate between the two groups (P>0.05). Conclusion Complications more frequently happen, operative trauma is larger, and 5-year survival is not increased after trans-thoracic cardiac cancer resection, though more esophagus and mediastinal lymph nodes can be removed. Therefore, trans-abdominal cardiac cancer resection is recommended.

    Objective To observe the effect of transcutaneous electrical acupoint stimulation (TEAS) and thoracic paravertebral block on sleep quality, pain, and quality of recovery after surgery in patients undergoing selective video-assisted thoracoscopic surgery. Methods A total of 60 patients undergoing video-assisted thoracoscopic surgery from June 2022 to March 2023 were selected and divided into the TEAS group or the control group by the random table method. Thirty minutes before anesthesia induction TEAS treatment was performed at Feishu, Neiguan, and Shenmen points bilaterally in the TEAS group until the end of surgery. The control group had the acupuncture electrodes on the same acupuncture point before anesthesia induction without electrical stimulus. The two groups of patients received a TPVB with ropivacaine (0.375%, 25 mL) before anesthesia induction. The visual analog scale (VAS) was used to evaluate pain scores after surgery and Pittsburgh Sleep Quality Index (PSQI) was used for evaluating subjective sleep quality. The PSQI scores were recorded at 24 h before operation (T0), 24 h (T1), 48 h (T2), 7 d (T3), and 14 d (T4) after operation. 15-item Quality of Recovery scores and VAS scores were also evaluated at 24 h after operation. Postoperative analgesic pump use and postoperative nausea and vomiting (PONV) were also recorded. Results Compared with the control group, the TEAS group showed significantly lower PSQI scores at T1, T2, and T3 (P<0.05), a higher QoR-15 score at postoperative 24 h (P<0.05), and lower VAS scores at T1, T2, and T3 (P<0.05). The frequency of analgesic pump pressing of the TEAS group was lower than that of the control group. In addition, the PONV score of the TEAS group was lower than that of the control group (P<0.05). There was no difference in other postoperative complications between the two groups (P>0.05). Conclusion TEAS combined with TPVB can relieve postoperative pain and improve quality of recovery of patients after video-assisted thoracoscopic surgery.

    Anterior cruciate ligament (ACL) is one of the most vulnerable structures during sports and an essential anterior stabilizing structure in the knee joint. ACL injury can impair a patient’s ability to move, and if not properly treated in time, it often results in serious adverse complications like meniscal and articular cartilage damage, and even traumatic osteoarthritis. Currently, ACL reconstruction is the standard treatment for ACL injury, and the choice of grafts has a significant impact on patients’ postoperative functional recovery and surgical success. Autografts are the most commonly used grafts and have the most stable efficacy, but they have drawbacks like limited sources and donor-site morbidity. Although having greater adaptability, allografts carry a risk of immune rejection and the spread of diseases. Artificial ligaments are appropriate for those who must quickly resume their athletic activities, but they are technically challenging and have not been shown to be long-term effective. Due to the numerous issues with traditional grafts, recent rapid advancements in tissue engineering techniques are generating novel ideas for ACL reconstruction. In order to aid in the treatment of ACL injury, this article discussed the benefits, drawbacks, and clinical effectiveness of different grafts used in ACL reconstruction.

    Clinical effect of laparoscopic surgery combined with methotrexate on ectopic pregnancy and prognosis

    Objective To investigate the clinical effect of laparoscopic surgery combined with methotrexate on ectopic pregnancy and prognosis. Methods A total of 90 cases of ectopic pregnancy who were admitted to Yangdong District People's Hospital of Yangjiang City from April 2018 to March 2021 were selected and divided into a control group and an observation group (45 cases each group) by random number table method. The control group received laparoscopic treatment, and the observation group received laparoscopic treatment in combination with methotrexate. Treatment effect, blood human chorionic gonadotropin (β-HCG) recovery time, postoperative abdominal pain rate, tubal patency, and hospital stay were observed in the two groups. At 12 months of follow up, natural pregnancy rate and recurrence rate of ectopic pregnancy were compared between the two groups. Results The total effective rate of curative effect in the observation group was higher than that in the control group (P<0.05). The β-HCG recovery time, postoperation abdominal pain rate, and length of hospital stay in the observation group were lower than those in the control group, and the tubal patency was higher (P<0.05). After 12 months of follow-up, the natural pregnancy rate was higher and the recurrence rate of ectopic pregnancy was lower in the observation group than in the control group (P<0.05). Conclusion Laparoscopic surgery combined with methotrexate in treating ectopic pregnancy is more effective than laparoscopic surgery only. It can shorten the time for blood β-HCG concentration to return to normal level, reduce postoperative abdominal pain rate, and improve the rate of tubal recanalization. The follow-up results suggest that laparoscopic surgery combined with methotrexate in the treatment of ectopic pregnancy can help improve natural pregnancy rate and reduce the recurrence rate of ectopic pregnancy.
