    Effect of extra-Glissonian transaction approach in laparoscopic hepatic left lateral lobectomy

    Objective To observe the effects of no blocking hepatic portal and intrahepatic extra-Glissonian transaction approach in laparoscopic hepatic left lateral lobectomy.Methods Patients who underwent laparoscopic hepatic left lateral lobectomy with no blocking hepatic portal and intrahepatic extra-Glissonian transaction approach and who underwent laparotomy from January 2016 to February 2017 were retrospectively analyzed for operation time,blood transfusion rate,blood loss,postoperative alanine aminotran sfease peak,postoperative complication rate,dietary intake time,abdominal drainage tube remove time,and postoperative hospital stay.Results The patients who had laparoscopic hepatic left lateral lobectomy with no blocking hepatic portal and intrahepatic extra-Glissonian transaction approach showed shorter dietary intake time,earlier abdominal drainage tube remove time,smaller postoperative ALT peak value,and shorter postoperative hospital stay than those who underwent laparotomy (P<0.05);no statistical difference in blood loss,blood transfusion rate,operation time,and postoperative complication rate were observed.Conclusion No blocking hepatic portal and intrahepatic extra-Glissonian dissection under laparoscope is an effective approach for laparoscopic hepatic left lateral lobectomy.

    Causes and surgical treatments of intraoperative massive haemorrhage in operation for primary liver cancer

    Objective To analyze common causes and corresponding treatments of massive haemorrhage in the operation for primary liver cancer.Methods Based on 14 cases of primary liver cancer with intraoperative massive haemorrhage,we summarized possible causes of intraoperative massive haemorrhage.Results Damages to hepatic short vein (5 cases),inferior vena cava (1 case),and right hepatic vein (2 cases) occurred during the separation of liver cancer.Damages to large intrahepatic venous branches (6 cases) occurred during hepatecomy.Conclusion Operation plans help avoid damages during the resection of large tumors in right liver and middle liver.In addition,ligating under direct vision (or after hepatic blood occlusion) rather than random clamp is recommended for intraoperative massive haemorrhage.

    Application value of breast plastic surgery in breast conserving surgery for breast cancer

    Objective To assess the value of breast plastic surgery in breast conserving surgery for breast cancer.Methods Study subjects included 98 cases with breast conserving surgery for breast cancer.According to the location and size of tumor,breast incision was designed to perform local extended resection,then repairment of breast defects was performed using self breast or external mammary tissue valve repair.Surgical procedures,positive margins,complications,and patients’ satisfaction were observed and recorded.Results Main surgical techniques performed for the patients included inverted-T superior pedicle technique,medial reduction,inverted-T inferior pedicle technique,J technique,lateral pedicle adipofascial technique,and periareolar incision technique.After surgery,17 patients (17.35%) showed positive margins and became negative after reoperation;13 cases developed complications,with a complication rate of 13.27%;the satisfaction rate of the patients was 88.78% (87/98).Conclusion Breast plastic surgery is effective for the patients with breast conserving surgery for breast cancer and could reduce complications and improve patients’ quality of life and satisfaction.

    Analysis on clinical characteristics and gastroscopy results of elderly patients with peptic ulcer

    Objective To investigate the clinical and gastroscopic features of peptic ulcer in the elderly,and to provide a basis for relevant diagnosis and treatment.Methods A total of 187 elderly cases with peptic ulcer and 239 control cases from January 2011 to December 2016 were enrolled in this study.Their clinical manifestations and gastroscopic results were analyzed.Results The clinical symptoms of the elderly cases with peptic ulcer were not typical,showing first signs of haematemesis and melena,while the controls showed typical symptoms.Drinking and taking nonsteroids anti-inflammatory drugs(NSAIDs) were the risk factors for peptic ulcer of the elderly patients,and there were significant differences in the rates of drinking and taking NSAIDs between the two groups (P<0.001).The elderly patients reported more gastic ulcers,the controls reported more duodenal ulcers,and there were significant differences in the incidence rates of the two diseases (P<0.001).A higher rate of large ulcers (diameter>2 cm) was found in the elderly patients than in the controls,and the difference was significant (P<0.001).Conclusion The clinical symptoms of peptic ulcer are not typical in the elderly patients.There are significant differences between the elderly patients and controls in lesion site,lesion size,and risk factors.Targeted diagnosis and treatment of peptic ulcer are required for the elderly.

    Comparative analysis of laparotomy and laparoscopic intersphincteric resection in treatment of stage Ⅱ-Ⅲ low rectal cancer

    Objective To analyze and compare the clinical effects of laparotomy and laparoscopic intersphincteric resection (ISR) in the preservation of rectal cancer at stage Ⅱ-Ⅲ.Methods A total of 70 patients with stage Ⅱ-Ⅲ low rectal cancer were selected from Department of General Surgery and randomly divided into an observational group (laparotomy ISR) and a control group (laparoscopic ISR).The clinical indicators,incidence rate of postoperative complications,and recurrence rate between the two groups were compared.Results The observation group showed significantly less intraoperative blood loss volume[(94.68±18.76) mL vs.(203.22±35.18) mL],shorter first anal exsufflation time[(1.21±0.36) d vs.(2.18±0.67) d],and shorter hospital stay[(9.54±1.39) d vs.(12.73±1.52) d] than the control group (P<0.05),but no differences in operation time and the number of lymph node dissection were observed.The incidence rate of postoperative complications (11.43% vs.37.14%) and recurrence rate (5.71% vs.22.86%) in the observation group were also significantly lower than those of the control group (P<0.05).Conclusion Compared with laparotomy ISR,laparoscopic ISR has a better clinical therapeutic effect and lower postoperative complication rate and recurrence rate;therefore,it is worthy of clinical application.

    Comparison of thoracic and transabdominal approches for the treatment of esophageal hiatal hernia

    Objective To compare the safety and effectiveness of thoracic approach and transabdominal approach for the treatment of esophageal hiatus hernia.Methods Eighteen patients with esophageal hiatal hernia who underwent surgical treatment were randomly divided into group A (10 cases with thoracic approach) and group B (8 cases with transabdominal approach).The perioperative clinical indicators and short-term follow-ups of the two groups were compared.Results All the operations were successful.One case with pulmonary infection and two cases with functional gastroparesis in group A were recovered after treatment and discharged from hospital.There were no statistically significant differences in operative time,incidence rate of postoperative complications,postoperative drainage,postoperative hospital stay,and total cost of hospitalization between group A and group B.After a 3-month follow-up,there was no obvious discomfort,and clinical symptoms were effectively relieved.Conclusion These two approaches for the treatment of esophageal hiatal hernia could achieve the same effect with good short-term prognosis.

    Application of painless treatment in perioperative period of anorectal surgery

    Objective To evaluate the effect of painless treatment in perioperative period of anorectal surgery.Methods A total of 200 patients with anorectal surgery were randomly divided into an experimental group and a control group.The experimental group (n=100) was treated with painless multimodal analgesia.The control group (n=100) was treated with oral celecoxib or intramuscular tramadol.The pain scores of postoperative 6 h,24 h,48 h,5 d,and first defecation,as well as the satisfaction score,of the two groups were compared.Results The pain score of postoperative 6 h of the experimental group was (3.6±0.6),and that of the control group was (3.7±0.3),showing no statistical difference (P>0.05).The pain scores of postoperative 24 h (2.7±0.9),48 h (4.3±0.4),5 d (3.1±0.6),and first defecation (5.7±1.5) of the experimental group were lower than those of the control group [(5.9±0.2),(5.6±1.3),(6.3±0.4),and (8.9±1.5),respectively],and the difference was statistically significant (P<0.05).The satisfaction score of the experimental group (4.6±0.3) was higher than that of the control group (3.4±0.6)(P<0.05).Conclusion Painless treatment for perioperative anorectal patients using multimodal analgesia can effectively reduce postoperative pain and improve quality of life.

    Curative effect of hip bath therapy with detumescence and analgesia prescription on perianal abscess

    Objective To explore the effect of hip bath therapy using detumescence and analgesia prescription after surgical incision and drainage of perianal abscess.Methods Patients treated with perianal abscess incision and drainage in Wuxi Hospital of Traditional Chinese Medicine in recent years were divided into two groups.The control group was treated with conventional therapy,while the observation group was treated with combined hip bath therapy using detumescence and analgesia prescription.Comparisons of the curative effects were made between the two groups.Results The total effective rate was 93.3% in the observation group,which was statistically higher than that in the control group (80.0%) (P <0.05).Shorter time of detumescence,wound exudate disappearance,wound pain relieve,dysuria disappearance,and wound healing was observed in the observation group than in the control group (P<0.05).In the follow-up visit 6 months after surgery,there were 3 cases and 7 cases of abscess occurred in the observation group and the control group,respectively,and the difference was statistically significant (P<0.05).There were 18 cases and 16 cases of anal fistula occurred in the observation group and the control group,respectively,with no difference between the two groups.The overall recurrence rate was 63.3% in the observation group,which was significantly lower than that in the control group (83.3%) (P<0.05).In addition,the overall satisfaction rate was 96.7% in the observation group,which was significantly higher than that in the control group (83.3%) (P<0.05).Conclusion After surgical incision and drainage of perianal abscess,conventional treatment combined with hip bath therapy using detumescence and analgesia prescription can effectively promote recovery,reduce recurrence rate,and improve curative effect and satisfaction of patients,which is worthy of clinical application.

    Comparison of clinical effects of two internal fixations on C3 distal radius fractures in the elderly

    Objective To explore the clinical effects of two internal fixations on C3 distal radius fractures in the elderly.Methods Thirty-eight elderly patients with C3 distal radius fractures received surgical treatment between October 2014 and April 2016.Among them,20 cases had internal fixation with locking volar plates,and 18 cases used traditional nonlocking volar plates and monolateral external fixators (the external fixators were removed two months later).The radiographs of fracture reduction,healing,and internal fixation sites,as well as clincal functions such as wrist function,at 3 and 12 months postoperative were followed and compared between the two groups.Mayo scores were used to evaluate wrist function.Results The 38 cases were followed up for an average of 16 months (between 12 and 20 months) after surgery.At 3 months postoperative,both groups healed without reduction lost or internal fixation loose,except that one case who used locking volar plates showed distal radius shortening and radial inclination loose.The mean Mayo score of the group using locking volar plates was 86.3±12.2:excellent for 10 cases,good for 9 cases,and satisfactory for 1 case,with an excellent-and-good rate of 95%;the mean Mayo score of the group using traditional nonlocking volar plates and monolateral external fixators was 81.9±14.3:excellent for 6 cases,good for 9 cases,and satisfactory for 3 cases,with an excellent-and-good rate of 83%;there was a statistical difference between the two groups (P<0.05).At 12 months postoperative,both groups healed well without reduction lost or internal fixation loose.The mean Mayo score of the group using locking volar plates was 93.2±10.3:excellent for 14 cases,good for 5 cases,and satisfactory for 1 case,with an excellent-and-good rate of 95%;the mean Mayo score of the group using traditional nonlocking volar plates and monolateral external fixators was 90.6±12.1:excellent for 11 cases,good for 6 cases,and satisfactory for 1 case, with an excellent-and-good rate of 94%;there was no difference between the two groups (P>0.05).Conclusion Both the studied internal fixations,locking volar plates and traditional nonlocking volar plates plus monolateral external fixator,are effective for C3 distal radius fractures of the elderly,but the former one has a better effect on the early recovery of wrist function.

    The research of concept of damage control orthopedics integrated with prehospital emergency treatment of pelvic fractures

    Objective To evaluate the effect of applying the concept of damage control orthopedics on prehospital emergency care of pelvic fractures.Methods From June 2014 to October 2016,a total of 128 patients with pelvic fractures were treated in Wuxi Emergency Hospital.Comparisons of curative effects were observed between patients treated with different prehospital emergency measures.Results The excellent rate of fracture reduction of the experimental group (92.2%,59/64) was significantly higher than that of the control group (75.0%,48/64) (P<0.05).The prognosis and Cole scores of the patients treated with the concept of damage control orthopedics were significantly better than those of the control group (such as mortality and complications) (P<0.05).Conclusion The application of damage control orthopedics concept can significantly improve the prehospital emergency treatment effect in patients with pelvic fractures before hospital admission,which is beneficial to patients’ prognosis and should be widely promoted.

    Objective To analyze the changes and trends of serum Ca2+ concentration in patients with different degrees of brain injury.Methods The serum Ca2+concentrations of 79 patients with acute traumatic brain injury and 30 healthy blood donors were determined by atomic absorption spectrophotometry.Then 71 out of the 79 patients were selected to determine the serum Ca2+ concentration after hospital admission.Results The patients with acute traumatic brain injury showed significantly lower serum Ca2+ concentration than the healthy controls.The more serious the injury was,the steeper the decline was.Conclusion More severe brain injury is assoicated with lower GCS score and lower serum Ca2+concentration.

    Objective To investigate the effect of extraction therapy on the vertical structure of craniomaxillofacial region in patients with high angle.Methods A retrospective analysis was conducted for 76 cases of high angle cases in orthodontics from February 2014 to August 2015 who were divided into two groups according to treatment regimens.The observation group (n=40) was treated with orthodontic premolars,and the control group (n=36) used using non-extraction treatment program.The changes of craniomaxillofacial vertical structure of the two groups by two different treatment regimens were measured using X-rays.Results After treatment,ANS-Me,L6-MP,N-Me and U6-PP in the two groups were significantly higher than those before treatment (P<0.05).There were differences in Y-axis and L6-MP between the two groups (P<0.05).Conclusion There is no significant change in the structure of mandibular plane angle and growth direction before and after orthodontic treatment.The height of molar is higher in the observation group than in the control group.

    Stem cells have the potential for self-renewal and multi-directional differentiation.Experiments have proven that stem cells can differentiate into hepatocyte-like cells under certain conditions,with hepatocyte markers and functions.Liver transplantation is the only effective and curative way for end-stage liver disease,but is limited by a shortage of donor organs,high costs of surgery,long-term immune therapy,and so on.In light of that,stem cells are of potential application value in the treatment of end-stage liver disease.This review focused on the regulatory mechanisms of stem cells differentiated into hepatocyte-like cells.

    Research progress on ultrasound-guided transversus abdominis plane block for abdominal surgery

    Ultrasound-guided transversus abdominis plane (TAP) block is a peripheral nerve (T6-L1) block for anterior abdominal wall.The method is injecting local anesthetics into the gap of fascia transversalis and deep muscle under ultrasound guidance.There are three major approaches used in clinical practice currently,including abdominal lateral approach,subcostal approach,and posterior approach.According to the level of anesthesia,we can choose corresponding approaches.TAP block can be used for anesthesia in abdominal surgery and postoperative analgesia.As one multimodal analgesia scheme,TAP block can improve analgesia effect.Ultrasound guidance can improve the success rate of TAP block and reduce the incidence rate of complications.The combined application of dexmedetomidine can enhance analgesic quality.Continuous TAP block can achieve sustained and stable analgesic effect.

    Nocturia is a very common but often overlooked disease,with a high morbidity.Previous studies have shown a correlation with depression,sleep disorder,mood disorder,and fall.The pathophysiological mechanism of nocturia is diverse and the definition is controversial.Its clinical manifestations are similar to nocturnal urination,enuresis,and urinary incontinence.The treatment for nocturia using a single medicine often fails to achieve desired effects,whereas combinations of drugs may be a new direction.This article reviewed the definition,etiology,differential diagnosis,and treatment of nocturia.

    Visceral hypersensitivity is the important pathogenic mechanism of irritable bowel syndrome.Psychogenic stress is not only the cause of irritable bowel syndrome,but also a regulator of the expression of brain-derived neurotrophic factor (BDNF) in brain regions.In animal models of psychogenic stress,BDNF regulates MAPK/ERK,CaMK,and PI3K/Akt signal pathways through combining with high affinity nerve growth factor receptor TrkB and is accompanied with the change of synaptic plasticity in many brain regions associated with the formation of visceral hypersensitivity.Here,we reviewed the relationship between BDNF and visceral hypersensitivity induced by psychogenic stress.

    Objective To analyze the influences of nursing dispute intervention on the development of medical work in Luoding People’s Hospital of Guangdong Province.Methods A total of 50 cases of nursing disputes that occurred from January 2014 to November 2015 were included in this study to analyze the influences of nursing dispute intervention on the development of medical work.Results Significant reduced nursing disputes were reported after the intervention implemented (P<0.05).Improper nursing observation was a major cause of nursing disputes.In addition,years of working was an influencing factor as a significantly lower incidence rate of nursing disputes was observed in the nurses with 6-10 years of service length than in those with 1-5 years of service length (P<0.05).Conclusion It is necessary to strengthen the nurses’ in-service training for improving their comprehensive quality and carry out related intervention programs for reducing the incidence rate of nursing disputes.Nursing quality management is of great importance to prevent nursing disputes and the smooth development of medical work.

    Objective To apply the combination of MOOC with traditional teaching mode to nurse licensure examination tutorial.Methods Nursing students (Class 2013) from a nursing college were divided into two groups.The control group adopted traditional teaching mode,and the experimental group adopted MOOC supplemented with traditional teaching mode.Results The students of the experimental group showed higher satisfactory rates and nurse licensure examination scores than the controls.Conclusion The MOOC-based teaching mode can improve students’ interests in learning as well as nurse licensure examination scores and pass rates.

    Objective To investigate the application of environment-friendly tracheotomy waterproof pad to patient care after tracheotomy.Methods Forty patients who underwent tracheotomy in the Department of Otolaryngology of Tongji Hospital,Tongji University School of Medicine from January 2015 to March 2017 were enrolled in this study.All subjects were randomly divided into an observation group and a control group with 20 cases in each group.The control was treated with regular moist gauze cover and aerosol inhalation,and the observation was additionally treated with environment-friendly waterproof pad.Comparisons were made between the two groups in the post operative usage rate of moist gauze during hospital stay,dampness of collar and gauze on the incision after aerosol inhalation treatment,and surrounding skin condition after tracheotomy.Results The usage rate of moist gauze in the control group was 70%,and the rate in the observation group was 85%.After aerosol inhalation,the incidence rate of dampness of collar and gauze was 95% for the control group and 15% for the observation group.In the control group,the incidence rate of skin swelling surrounding incision was 20%,and such symptom did not occur in the observation group.Conclusion Environment-friendly waterproof pad could increase patients’ adherence to moist gauze,avoiding damp collar and and decreasing skin swelling surrounding incisions.
