    Value of miR-155 and miR-21 in diagnosis of molecular subtypes of breast cancer

    Objective To investigate the expression and clinical significance of miR-21 and miR-155 in the molecular subtypes of invasive breast cancer.Methods The clinicopathologic data of 91 cases of invasive breast cancer were retrieved.The expression levels of miR-21 and miR-155 were detected by qRT-PCR.Results The relative expression levels of miR-21 in luminal A-like,luminal B-like,HER2,and TNBC (triple-negative breast cancer) subtypes of breast cancer were 0.000±2.238,1.008±1.297,1.670±1.774 and 0.983±1.630,respectively,and the relative expression levels of miR-155 were -2.313±1.077,-1.022±1.511,-0.051±0.903 and -0.728±1.585,respectively.There were significant differences in the relative expression levels of miR-155 of luminal B-like,HER2,and TNBC subtypes versus that in luminal A-like subtype (P<0.001).Conclusion Detecting the expression levels of miR-155 in invasive breast cancer may be a new approach for guiding breast cancer patients of luminal type to undertake chemotherapy or not.

    Effect of neoadjuvant chemotherapy on sentinel lymph node in breast cancer patients

    Objective To evaluate the detection rate,accuracy rate,and false negative rate of axillary sentinel lymph node biopsy (SLNB) after neoadjuvant chemistry (NAC) in axillary lymph node (ALN) clinical node negative (cN0) patients examined by physical examination,breast ultrasound,and breast magnetic resonance imaging (MRI),and discuss the feasibility of ASLNB after NAC.Methods Seventy-four cases of breast cancer admitted to the Department of Surgery Oncology,Provincial Clinical Institute,Fujian Medical University from August 2013 to July 2016 were enrolled in the study for retrospective analysis.All of the patients receiving NAC were divided into three groups according to the results of physical examination,breast ultrasound,and MRI.There were 55 cN0 cases judged by physical examination and breast ultrasound (group A),50 cyN0 cases by additional breast MRI (group B),and 24 clinical node positive (cN+) cases by all the three examination techniques (group C).Results In group A,the clinical remission rate of ALN was 74.3%,and the successful rate,accuracy rate,and false negative rate of ASLNB were 100%,92.7%,and 9.7%,respectively.In group B,the above indicators were 67.6%,100%,94.0%,and 7.8%,respectively.The cN+ patients in group C were given axillary lymph node dissection (ALND).The false negative rate of ASLNB had no significant difference (P>0.05) between group A and group B.Conclusion SLNB can replace ALND for the patients whose ALN transformed to cN0 from cN+ after NAC by physical examination and breast ultrasound.The false negative rate can be further decreased with additional MRI.MRI can be used to judge ALN in breast cancer patients after NAC.

    Observation of laparoscopic total mesorectal excision with pelvic automatic nerve preservation for patients with rectal cancer

    Objective To observe the clinical effect of laparoscopic total mesorectal excision (LTME) with pelvic automatic nerve preservation (PANP) on patients with rectal cancer.Methods A total of 82 patients with rectal cancer were selected from February 2012 to June 2015 in Luoding People’s Hospital.Of them,41 cases received open total mesorectal excision and PANP (OTME+PANP group),and the other 41 cases received LTME and PANP (LTME+PANP group).The operation condition,complication incidence,and survival rate within one year of each group were observed.Results The operation time,bleeding volume,recovery time of gastrointestinal function,time of leaving bed,and hospitalization time of the LTME+PANP group was significantly better than those of the OTME+PANP group (P<0.05); the incidence rate of complications of the LTME+PANP group was significantly lower than that of the OTME+PANP group (7.32% vs.21.95%,P<0.05); the recurrence rate (0%) and the survival rate (100%) within one year of the LTME+PANP group were not statistically different from those of the OTME+PANP group (2.44% and 100%) (P<0.05).Conclusion The combination of LTME and PANP has a better treatment effect for patients with rectal cancer,resulting in decreased postoperative complication incidence and faster recovery of gastrointestinal function,thus the surgery is worthy of clinical application.

    Clinical analysis of minimally invasive esophagectomy for right recurrent laryngeal nerve lymph nodes in 80 patients

    Objective To study the effect of minimally invasive esophagectomy (MIE) on right recurrent laryngeal nerve lymph nodes,and the characteristics of lymph node metastasis.Methods The clinical data of 80 cases of thoracic esophageal cancer and receiving MIE from January 2012 to June 2015 were analyzed retrospectively.Right recurrent laryngeal nerve lymph node metastasis and clinical pathological features were analyzed.Results (1) In all the patients receiving MIE,the total and average number of right recurrent laryngeal nerve lymph nodes were 322 and 4.0 per case (322/80),respectively,similar to the results by thoracotomy.Twenty-six cases were confirmed by pathology with right recurrent laryngeal nerve lymph node metastasis,and the metastasis rate was 32.5% (26/80).(2) Right recurrent laryngeal nerve lymph node metastasis was found in 21 (42.0%) patients with upper and middle segment esophageal cancer and 5(16.7%) with lower segment esophageal cancer,and the metastasis rate was higher in the patients with upper and middle segment esophageal cancer than in those with lower segment esophageal cancer (χ2=4.39,P<0.05).The metastasis rates in the well differentiated and moderately-poorly differentiated groups were 9.1% and 48.9%,respectively,and the difference was significant (χ2=12.27,P<0.01).The right recurrent laryngeal nerve lymph node metastasis rate was higher in the patients with vascular invasion (57.1%) than those without (25.8%) (χ2=3.94,P<0.05).Lymph node skip metastasis was found in 26 cases with lymph node metastasis.Conclusion In the patients with thoracic esophageal,right recurrent laryngeal nerve lymph nodes metastasis is common and the metastasis rate is closely related with tumor differentiation degree,tumor site,and vascular invasion.It is necessary to perform right recurrent laryngeal nerve lymph node dissection during thoracic esophagectomy.

    Comparative analysis on flexible ureteroscope lithotripsy and mini-percutaneous nephrolithotomy for upper ureteric calculi

    Objective To compare the effects and security of flexible ureteroscope lithotripsy and mini-percutaneous nephrolithotomy on upper ureteric calculi.Methods A total of 180 cases of upper ureteric calculi were selected from June 2015 to June 2017.Among them,90 cases were given flexible ureteroscopy lithotripsy with holmium laser as flexible ureteroscope group,and the rest 90 cases were given mini-percutaneous nephrolithotomy with holmium laser as mini-percutaneous nephrolithotomy group.The two groups were compared in operation time,blood loss volume during operation,stone clearance rate,postoperative hospital stay,BCS comfort score,VAS pain score,C reaction protein (CRP),white blood cell (WBC) count,and complications.Results There was no puncture failure or flexible ureteroscope lithotripsy failure cases in the two groups,and 3-6 d after operation,the positions of double J stents were normal.There was no significant difference in operation time,stone clearance rate,and WBC count between the two groups (P>0.05).In the flexible ureteroscope group,the blood loss volume during operation,postoperative hospital stay,VAS pain score,and CRP were lower,and the BCS comfort score was higher than those in the mini-percutaneous nephrolithotomy group,with significant differences (P<0.01).Two cases had fever in the flexible ureteroscope group and no conversions to open surgery.No serious complication was found in the mini-percutaneous nephrolithotomy group.Conclusion Compared with mini-percutaneous nephrolithotomy,flexible ureteroscope lithotripsy could bring more comfort,safety,and definite therapeutic effect for patients with upper ureteric calculi.

    Ultrasound diagnosis of diabetic lower extremity vascular lesions and its clinical value

    Objective To evaluate the ultrasound diagnosis of diabetic lower limb vascular lesions and its clinical value.Methods From January to December 2016,patients with diabetic lower extremity vascular disease (n=50) admitted to Xuyi County People’s Hospital of Huai’an City were enrolled as observation group,and non-diabetic patients (n=50) were enrolled as control group.Both groups received lower extremity vascular color Doppler ultrasound,and the ultrasonographic features of the two groups were compared and analyzed.Results Of the 50 observation cases,47 cases (94%) had vascular injury.Specifically,the Doppler ultrasound examination results showed plaque in 41 cases (41/47,87.23%),stenosis in 14 cases (14/47,29.79%),and luminal occlusion in 11 cases (11/47,23.40%).Of the 50 control cases,22 cases (44%) had vascular injury.Specifically,the Doppler ultrasound examination results showed plaque in 13 cases (13/22,59.09%),stenosis in 4 cases (4/22,18.18%),and no vascular occlusion.The positive rates of lower extremity vascular injury,vascular plaque,stenosis,and luminal occlusion were significantly different betweent the two groups (P<0.05).According to the lower extremity vascular Doppler ultrasound imaging,compared with the control group,the observation group showed luminal stenosis,blood flow reduction,and spectrum width broadening.Conclusion Ultrasonography shows a high detection rate of lower extremity vascular injury.The simple and non-invasive precedure provides important imaging evidence for early diagnosis and treatment for the disease.

    Comparison of clinical effects of minimally invasive surgery and traditional open surgery on lumbar degenerative diseases

    Objective To compare the clinical effects of minimally invasive transforaminal decompression of lumbar fusion with internal fixation and conventional posterior open surgery on lumbar degenerative diseases,and provide a basis for treatment of clinical lumbar diseases.Methods From January 2016 to January 2017,a total of 64 cases of degenerative lumbar joint discogenic back pain were recruited to observe the clinical effects of different treatments.Results The patients who had minimally invasive transforaminal decompression of lumbar fusion with internal fixation showed better results of length of incision,postoperative drainage volume,hospital stay,VAS score,and ODI score than those who had conventional surgery,and the differences were significant (P<0.05).Conclusion For lumbar degenerative diseases,minimally invasive transforaminal decompression of lumbar fusion with internal fixation can significantly improve operation completion and various postoperative scores,and is worth promoting.

    Clinical observation of conservative treatment and surgical treatment for distal radius fracture

    Objective To observe the difference in curative effects between surgical treatment and conservative treatment on distal radius fracture.Methods The hospitalized patients with distal radius fractures were randomly divided into a conservative treatment group and a surgical treatment group.Comparisons in the pain levels of fracture site and ulnar wrist and the medical imaging evaluation results were made between the two groups.Results The surgical treatment group showed a lower score of VAS pain in fracture site and ulnar wrist and a higher excellent and good rate of imaging evaluation than the conservative group,and the differences were statistically significant (P<0.05).Conclusion Surgical treatment for distal radius fractures is more effective than conservative treatment,with less postoperative pain and improved fixation effect,thus is worthy of clinical promotion.

    Observation of curative effect of distal radius fracture surgery and herbal fumigation on wrist joint function in elderly patients

    Objective To observe the effect of distal radius fracture surgery combined with herbal fumigation on wrist joint function in elderly patients.Methods Between January 2015 and May 2017,a total of 38 cases of distal radius fractures were divided into an observation group and a control group according to the random number table method,with 19 cases in each group.Both the two groups were treated with conventional distal radius fracture reduction and internal fixation surgery,as well as functional rehabilitation training conducted at 2 weeks after the operation.Additionally,the observation group was treated with herbal fumigation for 4 weeks and follow-up for 8 weeks.The wrist joint function recovery and fracture healing effect were compared between the two groups.Results The observation group showed better fracture healing effect,shorter healing time,and higher recovery rate than the control group,and the differences were statistically significant (P<0.05).Conclusion Herbal fumigation is beneficial to the healing of fracture and recovery of wrist joint function of the elderly patients after surgery.

    Clinical effect of large decompressive craniectomy on craniocerebral trauma complicated with intractable high cranial pressure

    Objective To explore the clinical effect of large decompressive craniectomy on craniocerebral trauma complicated with intractable high cranial pressure.Methods A total of 86 craniocerebral trauma patients complicated with intractable high cranial pressure treated from April 2014 to April 2017 were selected and randomly divided into two groups,with 43 cases in each group.The control group adopted conventional bone flap craniotomy.The observation group adopted large decompressive craniectomy.The clinical effects in the two groups were observed.Results The recovery rate of the observation group was significantly higher than that of the control group (P<0.05).After 3 months of surgery,the NIHSS score of the observation group was significantly lower than that of the control group (P<0.05).The incidence rate of complications of the observation group was significantly lower than that of the control group (P<0.05).Conclusion For craniocerebral trauma patients complicated with intractable high cranial pressure,large decompressive craniectomy can improve the clinical effect and neurological function,and reduce the incidence rate of complications.It is worthy of clinical promotion.

    Analysis on detection rate of transrectal ultrasound-guided prostate biopsy

    Objective To analyze the detection rates of various indicators at different levels following transrectal ultrasound-guided prostate biopsy,and evaluate the clinical value of ultrasound-guided biopsy.Methods Ninety-eight patients who underwent prostate biopsy guided by transtectal ultrasound (TRUS) from May 2009 to July 2016 were analyzed.Results In the ninety-eight patients,the detection rate of prostate cancer was 29.6%.The positive rates of biopsy increased with the rising of prostate specific antigen (PSA) levels (P<0.001).Ultrasound examination revealed nodules in outer glands of the prostate cancer cases.Conclusion The detection rate of prostate cancer is positively correlated with PSA level.Any outer gland hypoechoic nodules should arouse suspicion of prostate cancer.

    Objective To observe the clinical effect of bilateral intradermal thumbtack needle buried in Zusanli and Neiguan acupoints on nausea and vomiting in abdominal surgery patients.Methods A total of 160 patients receiving abdominal surgery were randomly divided into two groups:thumbtack needle group (group T,n = 80) and sham group (group S,n = 80).Both groups were anesthetized with sevoflurane-remifentanil combined anesthesia.At 30 min before surgery,the patients of group T underwent acupuncture treatment on bilateral Zusanli and Neiguan acupoints,and patients of group S were given sham treatment patches on the same acupoints.Nausea and vomiting responses were recorded within postoperative 48 h.Results Compared with the group S,the group T showed decreased postoperative nausea and vomiting degrees (P<0.05).Conclusion Applying intradermal thumbtack needle on Zusanli and Neiguan acupoints at 30 min before surgery can significantly reduce the incidence rate of nausea and vomiting after abdominal surgery.

    Distribution and antimicrobial resistance of 3302 pathogenic bacteria isolated from clinical samples

    Objective To understand the distribution and antimicrobial resistance of clinical bacteria,and provide scientific evidence for clinically reasonable use of antibiotics.Methods VITEK-2 Compact automated microbial identification system was used to identify clinical strains.WHONET 5.6 software was used to analyze 3 302 pathogenic bacteria isolated from clinical samples.Results A total of 3 302 strains of pathegenic bacteria were isolated.Of them,the prevalence of gram-negative bacteria was 73% (2 417/3 302) and that of gram-positive bacteria was 27% (885/3 302).The top five isolates were Escherichia coli (24%),Klebsiella pneumoniae (18%),Pseudomonas aeruginosa (10%),Staphylococcus aureus (9%),and Acinetobacter baumannii (9%).For Gram-negative bacteria,ESBLs-producing strains accounted for 50% in Escherichia coli,16.2% in Klebsiella,and 30.2% in Proteus mirabilis.The carbapenem-resistace of Acinetobacter baumannii and Klebsiella pneumonia was serious.For gram-positive bacteria,the prevalence of methicillin-resistant Staphylococcus aureus (MRSA) was 48.2%.The resistance rate of MRSA was higher than that of methicillin-sensitive Staphylococcus aureus (MSSA).Conclusion The bacterial drug-resistance is still a serious issue in clinical setting.Thus,more attention should be paid to multi-drug resistant pathogenic bacteria.Surveillance of bacterial resistance is of great significance to clinical infection control and rational drug use.

    NADPH oxidase-1 (NOX-1) is a member of nicotinamide adenine dinucleotide phosphate oxidase (NADPH oxidase,NOX) family.NOX is the key enzyme that regulates oxidation-reduction reaction in vivo and the earliest known system for producing reactive oxygen species (ROS).On the one hand,a small amount of ROS can enhance immune defense and participate in the regulation of cell differentiation,proliferation,apoptosis,and intercellular signaling pathways.On the other hand,excessive ROS can cause oxidative stress,resulting in cell physiological dysfunction,and play an important role in accelerating the pathogenesis of inflammation,fibrosis,and tumor formation.As one of the main sources of ROS,NOX-1 plays an important role in tumor formation.This paper reviewed NOX-1 and its roles and molecular mechanisms in tumors,in order to explore the effective targets for diagnosis and prognosis of malignant tumors such as gallbladder cancer.

    Optical coherence tomography angiography (OCTA) is a novel and non-invasive imaging technique that aquires fundus angiographic information with high resolution quickly,safely,and accurately,which strongly supports the diagnosis and treatment of various ocular diseases.This article reviewed the clinical application of OCTA to diabetic retinopathy (DR),including common changes of retinal capillaries and related OCTA images before and after anti-vascular endothelial growth factor (VEGF) medication.

    Dermatofibrosarcoma protuberans is a low-grade malignant fibrous tissue neoplasm with a low incidence,and primarily affects middle-aged men.Histopathological examinations are required to confirm diagnosis of dermatofibrosarcoma protuberans.The first choice for treatment is surgical resection,but the disease is subject to relapse.This article reported two female dermatofibrosarcoma protuberans patients with no recurrence after extended surgical resection.

    Role of quality control circle activity in improving efficiency of multiple injuries rescue

    Objective To study the role of quality control circle activity in improving the efficiency of multiple injuries rescue.Methods Since January 2016,a quality control circle activity related to “improving the efficiency of multiple injuries rescue” had been implemented in the Department of Emergency,Tongji Hospital Affiliated to Tongji University.After research on the status quo of the activity of quality control circle and clinical problem analysis,corresponding measures were formulated and implemented.Results After the activity,the efficiency of multiple injuries rescue was improved.The successful rescue rate and rescue time were statistically different between the two groups (P<0.05) using SPSS 20.0 statistical software.Conclusion After the quality control circle activity,the duration of multiple injuries rescure is decreased,the efficiency of rescue is increased,the procedure of multiple injuries rescue is improved,the quality of first aid is elevated,and the clinical ability of nurses is improved.

    Comparison of subcutaneous injection of compound anisodine in temporal region using different postures

    Objective To investigate the influence of different postures on the patients administered with compound anisodine injection in temporal region.Methods A total of 97 patients who required continuous temporal subcutaneous injection of compound anisodine for 14 days were randomly divided into a sitting group (injected with sitting posture,48 cases) and a lying group (injected with supine posture,head to healthy side,49 cases).The two groups of patients were compared in pain,negative emotion degree,adverse reaction rate,and satisfaction after first injection.Results The lying group showed remarkably lower pain level,negative emotion degree,and adverse reaction rate,and higher satisfaction than the sitting group.Conclusion In temporal subcutaneous injection of compound anisodine,using supine posture can reduce patients’ pain,adverse reaction rate,and the incidence of negative emotion,as well as improve patients’ compliance and satisfaction.

    Objective To investigate the influence of early diet after routine gastroscopy on patients’ comfort and safety.Methods A total of 900 patients with routine gastroscopy were randomized into experiment group 1, experiment group 2, and control group, with 300 cases in each group. The two experiment groups who did not cough after drinking a little warm water started diet at 60min (experiment group 1) and 90 min (experiment group 2) later, respectively, while the control group started at 2 h after inspection.Results The experiment group 1 showed a lower incidence rate of adverse reactions before eating than the control group. The experiment group 2 had little difference in the incidence rate of adverse reactions compared with the control group. There were no difference in nausea, vomiting, abdominal pain, cough, aspiration, and other adverse reactions among the three groups after eating.Conclusion Setting the diet time at 60 min after routine gastroscopy can obviously both improve patients’ comfort and ensure their safety.

    Research progress on treatment and care of urinary incontinence in perioperative elderly patients with orthopedic disease

    The incidence of urinary incontinence is on the rise in the world,but there are few studies in orthopaedic field.Perioperative elderly patients with orthopedic disease have little understanding of and pay little attention to urinary incontinence.Therefore,it is imminent to strengthen the research of urinary incontinence in orthopaedic perioperative elderly patients.This article reviewed the causes,types,and epidemiological information of urinary incontinence in elderly patients with orthopedics disease,and also summarized and analyzed the risk factors of urinary incontinence and major treatment and nursing measures,aiming to provide guidance to take interventions according to risk factors against urinary incontinence in perioperative elderly patients with orthopaedic disease in the future.
