    Renal cell carcinoma is a common type of urinary cancer with insidious onset and rapid progression. Advanced renal cell carcinoma has poor prognosis and primarily depends on systemic therapy which mainly composed of targeted therapy and immunotherapy. Recently, new targeted drugs, immuno-targeted combination therapy, HIF-2α inhibitors, and probiotic combination therapy provide more possible options for the treatment. In this review, we gave an overview of the advances in systemic therapy of advanced renal cell carcinoma, aiming to help make clinical decisions.

    In recent years the application of thermal ablation (TA) in the treatment of benign and malignant thyroid tumors has drawn wide attention. Many studies have shown that compared with traditional surgery, TA is a minimally invasive and cosmetic surgery. In particular, the safety and effectiveness of TA in the treatment of benign thyroid tumors have been generally proven, but are still controversial in the treatment of papillary thyroid microcarcinoma. This article reviewed related literature, and focused on the principles of different thermal ablation techniques, indications and contraindications for the treatment of benign and malignant thyroid tumors, the safety and effectiveness of TA, and the advantages and disadvantages of TA compared with traditional surgery.

    Anterior cruciate ligament (ACL) is one of the most vulnerable structures during sports and an essential anterior stabilizing structure in the knee joint. ACL injury can impair a patient’s ability to move, and if not properly treated in time, it often results in serious adverse complications like meniscal and articular cartilage damage, and even traumatic osteoarthritis. Currently, ACL reconstruction is the standard treatment for ACL injury, and the choice of grafts has a significant impact on patients’ postoperative functional recovery and surgical success. Autografts are the most commonly used grafts and have the most stable efficacy, but they have drawbacks like limited sources and donor-site morbidity. Although having greater adaptability, allografts carry a risk of immune rejection and the spread of diseases. Artificial ligaments are appropriate for those who must quickly resume their athletic activities, but they are technically challenging and have not been shown to be long-term effective. Due to the numerous issues with traditional grafts, recent rapid advancements in tissue engineering techniques are generating novel ideas for ACL reconstruction. In order to aid in the treatment of ACL injury, this article discussed the benefits, drawbacks, and clinical effectiveness of different grafts used in ACL reconstruction.

    ISG15 and breast cancer

    Breast cancer has become the most common women malignant tumor in the world. Nowadays, the diagnosis and treatment of breast cancer has gradually changed from traditional treatment methods to individualized and precise treatment methods. Interferon-stimulated gene 15 (ISG15) is related to the occurrence and development of many malignant tumors, and participates in tumor-related immune responses and the resistance of many chemotherapeutics. ISG15 can promote breast cancer cell movement, and participate in breast cancer-related tumor immunotherapy and various signaling pathways. These research findings provide new insights for breast cancer treatment. In this paper, the structure and function of ISG15, and its correlation with tumors, especially with breast cancer, were reviewed.

    Fractures of lunate bone are rare injuries of the wrist that are usually resulted from high-energy trauma and are typically accompanied by other carpal injuries, such as scaphoid bone, capitate bone, radial styloid, and distal radius fracture. Misdiagnosis,missed diagnosis or inappropriate treatment in clinical practice lead to a high nonunion rate of lunate fractures, which often affects the movement of the wrist and even leads to complications such as osteonecrosis or loss of wrist function. The potential pathogenesis, therapeutic method,and prognosis of lunate fractures are still controversial. This article reviewed domestic and foreign literature on the classification, imaging characteristics and treatment methods from the perspectives of anatomical characteristics and injury mechanism of the lunate, so as to provide guidance for improving the prognosis of lunate fractures.

    Rotator cuff tear is a common shoulder disease, especially among athletes and middle-aged and elderly individuals. With the accelerating trend of aging population, the number of patients with rotator cuff tears is increasing. For massive rotator cuff tears, there is a high risk of re-tear after surgery and poor prognosis, which poses a significant clinical challenge. In recent years, surgical techniques for massive rotator cuff tears have continued to innovate, including superior capsule reconstruction, subacromial balloon spacer implantation, tendon transfer, and reverse shoulder arthroplasty. This article summarized the biomechanical effects of massive rotator cuff tears on the shoulder joint and focused on the latest treatment advancements, aiming to provide new reference for clinical treatment.

    The disposal of medical supplies plays a very important role in public health emergency logistic support. As one of the important parts in epidemic prevention and control, the disposal of medical materials after the closure of large shelter hospitals is crucial for the full utilization of medical equipment and medical consumables, the proper placement of state-owned assets, and the conservation of medical resources. This paper summarized the practical experience in the disposal and management of medical materials after the closure of the Fangcang Shelter Hospital of the National Convention and Exhibition Center (Shanghai), focused on the principle of "overall allocation, full utilization, step-by-step implementation, and combination of use and storage", and formed a material disposal management system through the establishment of material disposal organization structure, division of responsibilities, and operation mechanism, in order to provide reference for further improving the management system of medical material disposal in public health events.

    With the continuation of the COVID-19 epidemic, China attaches more and more importance to responding to public health emergencies, and epidemic prevention and control as well as standardized and effective disinfection methods have become an immediate need for hospitals. Thanks to the development of science and technology, intelligent disinfection robots have become one of the options for hospital disinfection operations. This paper explores the feasibility of intelligent disinfection robots as a new approach to hospital disinfection by comparing the methods of manual disinfection versus intelligent robot disinfection, and using scenario simulation experiments to demonstrate the effect of robot disinfection.

    Objective To explore the effect of centralized management on reducing physical restraint in neurosurgical patients. Methods Neurosurgical patients with physical restraint were selected by purposive sampling method to implement the strategy of cluster management. Before and after comparisons were made in terms of the patients’ physical restraint rate, restraint-related complications, and unplanned extubation rate, and the nurses’ knowledge, attitude, and practice of physical restraint. Results There were significant differences in the rates of physical restraint, restraint-related complications, and unplanned extubation in neurosurgical patients and the scores of knowledge, attitude, and practice of physical restraint in nurses before and after the implementation of centralized management strategy of physical restraint (P<0.01). Conclusion The strategy of centralized management of physical restraint can reduce the incidence rate of physical restraint, restraint-related complications, and unplanned extubation in neurosurgical inpatients, improve nurses’ knowledge, attitude, and practice of physical restraint, ensure nursing safety, and increase the quality of nursing care.

    Objective To investigate the application value of cellular antomatic DNA image cytometry (DNA-ICM) in cervical cancer screening. Methods Ten cases of normal cervical tissues with diploid DNA content were selected as the control group; 69 cases of no intraepithelial or malignant lesions (NILM), 30 cases of low grade squamous intraepithelial leisons (LSIL), 65 cases of high grade squamous intraepithelial leisions (HSIL), and 28 cases of squamous cell carcinoma (SCC) were selected as the experimental groups. Pap staining cytological screening, automatic DNA-ICM and ploidy analysis after Feulgen staining, and Ki-67 immunocytochemical staining were performed. Results The sensitivity of DNA-ICM and HPV SIL+ screening was 65.9% and 84.6% (χ2=34.217, P<0.001), and the specificity was 72.5% and 55.1% (χ2=10.340, P = 0.006) respectively. The DI thresholds of NILM, LSIL, HSIL, and SCC were ≤4, ≤4, ≤6, and ≤5, respectively. The average number of anploid cells was 0.9, 3.9, 9.4, and 12.0, and the average number of Ki-67 positive cells was 3.1, 10.5, 88.6, and 255.6, respectively, with a significant difference between the two groups (χ2=14.680, P<0.05). The proportion of few (≤2) anploid cells was 10.1%, 33.3%, 26.1%, and 28.6%, respectively. The detection rates of cell DNA heteroploidy in the low, medium, and high risk HPV groups were 12.1%, 57.5%, and 82.3%, respectively, with a significant difference among the groups (χ2=43.420, P<0.001). Conclusion DNA index (DI) reflects the structural and quantitative changes of DNA during the cell cycle. The number and DI threshold of DNA anploid cells have important application values for cytological diagnosis, evaluation of proliferation activity and progression of lesions.

    Objective To summarize the experience of clinical research information management platform based on integrated digital thinking mode. Methods The links of project approval, start-up preparation, project implementation, quality supervision, and conclusion were connected, and then the informatization and standardization of the whole process were realized in combination with the guidance of clinical research at all levels. Results The construction of the clinical research information management platform provided high-quality data assets and helped hospital digital transformation. Conclusion The digital clinical research management platform has greatly standardized the process of clinical research in the hospital, realized efficient management of clinical research, and promoted the medical quality, medical scientific research exploration, and the construction and development of clinical disciplines.

    Comparison of alfentanil and sufentanil combined with propofol anesthesia in painless bronchoscopy

    Objective To observe and compare the anesthesia effects of alfentanil and sufentanil combined with propofol in painless bronchoscopy. Methods A total of 80 patients who underwent painless bronchoscopy from March 2021 to July 2021 were selected. According to random number table method, they were divided into a control group and an observation group, 40 cases in each group. Sufentanil was used in the control group (0.2 μg/kg), and at alfentanil in the control group (20 μg/kg). Mean blood pressure (before induction of anesthesia, 3 min after induction, and the end of the examination), wake-up time, orientation recovery time, respiratory depression, coughing and other adverse reactions were recorded in the two groups. Results The mean blood pressure levels of the observation group 3 min after induction and after examination in the observation group declined less than those in the control group (P<0.05). The wake-up time and orientation recovery time of the observation group were shorter than those of the control group (P<0.05). Conclusion Painless bronchoscopy using alfentanil combined with propofol anesthesia results in quick recovery time, stable hemodynamics, and short recovery time for orientation.

    Objective To explore the impact of virtual reality technology on cognitive impairment and daily living ability of patients in the rehabilitation treatment of hemiplegia after stroke. Methods A total of 80 patients with post-stroke hemiplegia who received treatment from September 2021 to March 2023 were selected as research subjects and randomly devided into two groups, namely the conventional group and the virtual technology group, with 40 cases in each group. The former received a conventional rehabilitation training plan, while the latter used virtual reality technology on the basis of the conventional rehabilitation training plan. The scores of the Montreal cognitive assessment scale (MoCA) and the activities of daily living (ADL) ability were compared between the two groups before and after 8 weeks of treatment. Results After the intervention, the MoCA and ADL scores of the virtual technology group were significantly higher than those of the conventional group (P<0.05). Conclusion Virtual reality technology can effectively alleviate cognitive impairment and enhance patients' daily living abilities in the rehabilitation treatment of hemiplegia after stroke.

    Application of "tension-compression balance reconstruction" theory in the treatment of intertrochanteric fractures

    Intertrochanteric fractures are very common, and internal fixation has become a consensus, which can reduce bedridden complications. Although the current internal fixation system has been gradually improved, there are still many reports of failure. Considering the structure and biomechanical characteristics of the proximal femur, the special structure of the proximal femur is maintained by tension trabecula and compression trabecula together. The effects of compression and tension trabecula disappear when intertrochanteric fractures occur, and the normal morphology of the proximal femur couldn’t be maintained. Current internal fixation can reconstruct the support structure of the proximal femur, which restores the function of compression trabecula, but neglects the function of tension trabecula. Overly relying on internal structure support is not a perfect internal fixation. Therefore, a better internal fixation should be able to reconstruct both tension and compression trabecula, ensuring tension-compression balance and joint function, maintaining the stability of the affected side and the morphology of the proximal femur. We proposed for the first time the internal fixation theory of "tension-pressure balanceable reconstruction" for the treatment of intertrochanteric fractures, aiming to provide a new direction of the internal fixation design for proximal femoral fractures and address the shortcomings of current internal fixation devices.

    Safety and efficacy of radiofrequency ablation for hyperthyroidism

    Objective To explore the safety and efficacy of ultrasound-guided radiofrequency ablation (RFA) in the treatment of hyperthyroidism. Methods Twenty-seven patients with hyperthyroidism from The Second Affiliated Hospital of Xiamen Medical College in the period of July 2018 to June 2019 were treated with ultrasound-guided RFA. Thyroid volume, total triiodized thyroid hormone (TT3), free triiodized thyroid hormone (FT3), total tetriodized thyroid hormone (TT4), free tetriodized thyroid hormone (FT4), thyroid stimulating hormone (TSH), clinical symptoms, and complications were compared before and after RFA. Results The clinical symptoms of patients were significantly improved after surgery (P<0.05). The thyroid volumes were significantly reduced 3, 6, 12 months after surgery (P<0.05). Serum TT3, TT4, FT3, and FT4 levels decreased significantly and TSH increased significantly after surgery (P<0.05). The hoarseness rate reached 3.7% and tachycardia rate reached 11.1%, which all recovered after treatment without recurrence and no other complications occurred. Conclusion Ultrasound-guided percutaneous RFA is a safe and effective method for hyperthyroidism, which can improve the symptoms significantly with few complications.

    Portal vein tumor thrombus is a typical clinical manifestation of advanced hepatocellular carcinoma(HCC), and it is also the main cause of death. In recent years, the idea and strategy of diagnosis and treatment of HCC complicated with portal vein tumor thrombosis have changed greatly with the successive use of targeted therapy and immunotherapy, such as the classification and staging of portal vein tumor thrombus, the process of multidisciplinary diagnosis and treatment, and the combination of comprehensive treatment.

    To summarize the perioperative nursing experience of a case of aortic sinus tumor breaking into the right atrium. Provide comprehensive support and monitoring for patients with cardiac function before operation, and nursing intervention is carried out from the aspects of condition observation, hemodynamic monitoring, infection prevention, nervous system nursing, respiratory system nursing and health guidance after operation. After active treatment and care, the patient recovered successfully one week later.
