    The preliminary study of apoptosis and calcium metabolism in human colorectal cancer cells HCT 8 treated with Bufalin

    Objective:To study the influence and mechanism of bufalin on apoptosis and calcium metabolism in human colorectal cancer cell HCT 8. Methods:We used the method of CCK-8 to detect the proliferation of HCT 8 treated with bufalin of different concentrations at 24 hours or the same one at different time. And then , the apoptosis of the cell with different concentrations was detected by Annexin V / PI double staining apoptosis kit. Finally, we investigated the effect of different concentrations of bufalin on intracellular Ca2 + by using Ca2+ fluorescence probe Fluo-3 AM to mark its intracellular Ca2+. Results:The inhibition of human colorectal cancer cell HCT 8 with bufalin showed dose and time dependent manner obviously by the method of CCK-8. The result of apoptosis with flow cytometry illustrated that bufalin can induce apoptosis in the cell HCT 8, and represented a dose-dependent manner. According to the results of the intracellular Ca2+ labeled by calcium fluorescent probe calcium, there was a higher concentration of intracellular calcium when the cell treated at a higher concentration of bufalin. Conclusion:Bufalin can induce apoptosis in the cell of HCT 8, and it may be related to the metabolism of intracellular Ca2+.

    Use multiple-image fusion method to analyze morphological characteristics of radial head fractures

    Purpose:To measure the locationof fractures radial head using quantitative 3-dimensional computed tomography (CT). Materials and methods: We measured the fracture lines with respect to the biceps tuberosity in 24 patients with a displaced articular fracture of part of the radial head (12 terrible traid injury and 11 other injury). Results: 23 fractures created 43 fragments (average 1.9). In terrible traid injury, 12 fractures created 26 fragments, (average 2.2); In other injury cases, 11 fractures created 17 fragments (average 1.5). And the scattergram was created to show the centers of fragments. Conclusion: This quantitative analysis of CT scans of displaced articular fractures radial head confirms that the most common location is the anterolateral quadrant with the forearmin neutral rotation. Given the important role of the radial head in elbow stability, more accurate characterization of incomplete radial head fractures may improve our understanding of treatment and outcome of these fractures.

    Application value of fiberoptic ductoscopy in nipple discharge combined with intra-duct space occupying lesions

    Abstract Objective:To investigate the diagnosis value of fiberoptic ductoscopy (FDS) in nipple discharge combined with intra-duct space occupying lesions so as to improve diagnostic accuracy of the condition. Methods:The clinical data of 85 patients with nipple discharge who received FDS in our hospital from 2012.4 to 2016.6 were analyzed retrospectively, and the results were compared with the pathological examination. Result:In 85 patients with nipple discharge, 70(82.4%) cases of intra-duct space occupying lesions was found by FDS, including 10 cases of ductal breast cancer, 51 cases of papillomatosis, 9 cases of papilloma. Much performance of intra-duct space occupying lesions was hemorrhagic discharge (83.0%). The sensitivity and specificity of FDS was 90.0%, 95.0%, the coincidence rate was 94.3%. Duct grade of ductal carcinoma was usually Ⅱ~Ⅲ(80.0%). Conclusions: FDS has high diagnostic value of intra-duct space occupying lesions for nipple discharge without palpable mass, can definite location and characteristics of lesions, is helpful for early treatment.

    Comparative efficacy of radiofrequency ablation of small hepatocellular carcinoma TACE combined with sequential treatment with surgical resection

    SummaryObjective To explore the hepatic artery chemoembolization combined with radiofrequency ablation difference in efficacy of sequential therapy and conventional surgical resection alone small hepatocellular carcinoma.Methods 50 cases in January 2011 - February 2014 to our hospital patients with small hepatocellular carcinoma as the observed object, and randomly divided into two groups, one group received conventional surgical resection of small hepatocellular carcinoma, and the other group received TACE radiofrequency ablation, different ways were compared after 1 year, 2 years, 3-year survival rate of tumor recurrence, and its associated complications.Results Two groups of patients after treatment in different ways, the observation group after treatment of complications was significantly less than the control group (P <0.05), the cumulative relapse rate after treatment 1, 2, 3 years, the observation group: 12.0%, 16.0%, 32.0 % in the control group: 12.0%, 16.0%, 32.0% (P> 0.05); after treatment 1, 2, 3-year survival rate of the control group: 92.0%, 76.0%, 56.0% in the observation group was: 92.0 %, 80.0%, 60.0% (P> 0.05).Conclusion TACE combined with RFA sequential treatment of small hepatocellular carcinoma patients, efficacy, no significant difference in long-term prognosis with surgery, fewer complications, can be used as one of small hepatocellular carcinoma patients preferred.

    Application of Bidirectional Absorbable Barbed Suture in Double-Ring Breast Reduction Mastopexy

    Objective: We aimed to prove the value of bidirectional barbed absorbable suture (QuillTM SRS suture) in periareolar mastopexy. Methods: A total of 17 cases with breast ptosis were enrolled in this study who were subjected to continuous suturing on double-circle incision designed preoperatively with QuillTM SRS suture. Compared with previous 21 cases subjected to interrupted suture with conventional nonbarb absorbable filament, suturing time, wound healing and scar appearance in 6-month follow up were studied in both groups. Results: The time of suturing and wound healing was significantly short in QuillTM SRS group compared with conventional suture group (P < 0.05). Moreover, there was obvious significance of scar appearance in 6-month follow up between two groups. Conclusion: The use of QuillTM SRS suture in periareolar mastopexy can significantly decrease suturing time, improve the long-term quality of high-tension scars by more effective tension unloading and reduce the incidence of hyperplasia of scar. Therefore, Quill SRS suture will be widely applied in clinical treatment as a new suture material.

    The application of early enteral nutrition support in the treatment of acute pancreatitis

    Objective To explore the application of early enteral nutrition support (EEN) in the treatment of acute pancreatitis (AP). Methods The clinical data of 76 AP patients with EEN from January 2014 to June 2016 in our hospital were retrospectively analyzed. The time of starting EEN, the feeding way, and the tolerance were recorded. The incidence of EEN related inhalation pneumonia, hospitalization time and prognosis were measured. Results In this series, there were 5 cases of mild AP, 24 cases of moderate AP, 47 cases of severe AP. The time of starting EEN within 48h of admission in 25 cases, and 48~72h after admission in 51 cases. EEN feeding through nasojejunal tube in 46 cases (NJ group), and nasogastric tube in 30 cases (NG group). The intolerance of EEN occurred in 10 patients. Inhalation pneumonia occurred in 2 patients. The length of hospital stay was 14~35d with an average of 28.5±7.5d. 71 cases were cured, and 5 cases died. There was no significant difference in tolerance, inhalation pneumonia, length of stay and prognosis between NJ group and NG group (P>0.05). Conclusions EEN is helpful to improve the prognosis of patients with moderate and severe acute pancreatitis. Clinical factors such as the time of the feeding start and the gastrointestinal function should be considered for the EEN feeding through nasojejunal or nasogastric tube.

    [Abstract] ObjectiveTo investigate the clinical efficacy of suture-fixationfor the old injury of distal tibiofibular syndesmosis treated with Knotless TightRope.Methods52 consecutive patients undergoing surgery were divided into experimental and control groups.The experimental group was treated by Knotless TightRope, Control group was treated with 4.5mm screws. Postoperative complications, motion range of ankle-joint,VAS score, AOFAS score, SF-36 scores were recorded.Result Average follow-up period of 52 patients is 22.3 months. All scores have no significant difference after 6 months follow-up, but there is statistical difference in 22 months. All cases healed without infection. In experimental group,no re-seperation was observed.In control group,distal tibiofibular syndesmosis seperated in 5 patients after fixation breakage.ConclusionKnotless TightRopehas definite clinical effect to for the old injury of distal tibiofibularsyndesmosis.Reliable fixation has the advantages of early exercise with full weight bearing.

    Combination of Brostrom procedure and ankle arthroscopic for lateral ankle instability associated with intra-articular lesions

    [Abstract]Objective To evaluate the surgical outcome of Brostrom procedure assisted with arthroscopic. Method This is a retrospective study involving 32 patients who underwent Brostrom procedure assisted with arthroscopic for lateral ankle instability at Tongji Hospital, 21 patient is completed follow-up from December 2010 to December 2014. The patients consisted of 16 males and 5 females with an average age of 23.5 years. The American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Score was used to assess the pain and functional status in preoperative examination and postoperative follow-up at 3 months, 6 months,12 month and 18 months. Results Via Intra-operative ankle arthroscopic we found 19 patients associated with intra-articular lessions, presenting as synovitis in 12 cases,osteochondral lesion in 6 cases, osteophyte formation in 4 cases, soft tissue impingement in 3 cases, and syndesmosis widening in 1 case. The preoperative AOFAS score was 51.31±17.19. The AOFAS score for postoperative follow-up at 3 months was 77.40±9.99,6 months was 91.55±6.91, 12 months was 96.46±5.30, and 18 months was 97.15±3.08. Conclusion The Brostrom procedure assisted with ankle arthroscopic treatment for lateral ankle instability is a safe and effective method offering satisfying surgical outcome.

    Clinical evaluation of absorbable implants in treatment of ankle and foot fractures

    Objective To evaluate the effects of absorbable implants in treatment of ankle and foot fractures. Methods A total of 76 cases with ankle and foot fracture were operated with absorbable implants from June 2010 to June 2014. There were 47 males and 29 females,with average age of 46.5 years(18~63 years).There were 25 ankle frctures, 6 calcaneal and 45 metatarsal fractures. Results All cases were followed up for 12 to 49 months(average 22.4 months). All fractures healed. The rate of excellent and good clinical results according to AOFAS system was 96%. The incidence of complications was 4%, all were foreign-body reactions. Conclusion The absorbable implant was suitable for ankle and foot fractures. Appropriate indication and operative procedure were important for the successful treatment.

Bronchoscopy showed multiple nodules at the opening of the bronchus. Biopsy of the nodule showed submucosal ossification, lymphocyte infiltration and squamous metaplasia of the Surface respiratory epithelium with mild atypical hyperplasia .Conclusion TO is a rare disease. High resolution CT has significant clue to the diagnosis and bronchoscopy revealed characteristics. Its diagnosis should combined with pathological biopsy.

    Clinical application of a novel endoscopic mask used in aged outpatients undergoing painless gastroscopy

    [Abstract] Objective To evaluate the safety and effects of a novel endoscopic mask used in outpatients aged over 80 years undergoing painless gastroscopy. Methods 35 aged outpatients undergoing painless gastroscopy were managed with 5L/min oxygen providing by a novel patented endoscopic mask and intravenous anesthesia by propofol. Pulse oxygen saturation(SpO2), heart rate(HR) and mean arterial pressure(MAP) were recorded before, during and after the operation, as well as basic datas. Adverse reactions were also recorded. Results SpO2 during the procedure were significant higher than basic SpO2 (P<0.001), and no obvious desaturation was recorded. MAP decreased after anesthesia(P<0.05), but decreasing value of blood pressure was less than 20% of basic mean arterial pressure. HR remained stable and showed no significant difference. All patiengts went back to ward or home safely after a short break. Conclusions The use of the endoscopic mask in aged outpatients during painless gastroscopy is effective and safe, which could increase fraction of inspired oxygen and provide adequate oxygen supply, showing high clinical applicable value.

    Application of color Doppler ultrasound in diagnosis and differential diagnosis of abdominal masses in children

    To study the application of color Doppler ultrasound in the diagnosis and differential diagnosis of abdominal masses in children. Methods The clinical data of 40 children with abdominal mass confirmed by pathology in our hospital between May 2014 and May 2015 were retrospectively analyzed. All the children were diagnosed by color Doppler ultrasound. The diagnostic results of color Doppler ultrasound were analyzed. Results The results of color Doppler ultrasound diagnosis showed that among the 40 cases, 30 cases were definitely diagnosed, 3 cases were misdiagnosed and 7 cases were indefinitely diagnosed. Among them, there were 9 cases with nephroblastoma, and 7 cases were in accordance with the pathological findings, 8 cases were diagnosed as renal space occupying lesions before operation. Among them, there were 6 cases with hepatoblastoma, 5 cases were confirmed, 1 case was misdiagnosed as hepatoblastoma; there were 8 cases with severe hydronephrosis, mainly induced by congenital ureterostenosis, deformity and other reasons, of whom the causes of 2 cases with water were unclear, and 6 cases were confirmed; there were 11 cases with congenital bile duct cyst, the cause of 1 cases was not clear, and 10 cases were confirmed; there were 2 cases with neuroblastoma, 1 case did not indicate the source, 1 case was confirmed and 1 case was with repeated kidney. Preoperative ultrasound only indicated hydronephrosis, and it was not confirmed; there were 3 cases with teratoma, including 1 case with ovarian teratoma, 1 case with sacral tail teratoma and 1 cases with retroperitoneal teratoma. Among them, the origin of 1 case was unknown, and 2 cases were confirmed, and after surgery, they were confirmed as sacral tail teratoma. Conclusion Ultrasound diagnosis is quickly, simple to operate, safe and non-invasive, with low cost and high diagnostic rate. It can be used for the diagnosis of abdominal masses in children, and the clinical application value is high, especially in pediatric surgery, it has obvious advantages.

    Research Progress On Hip-reserving Surgerys Of The Femoral Head

    Osteonecrosis of the femoral head (ONFH), also known as femoral head avascular necrosis, is a pathological state due to different etiologies, including steroid administration, alcohol abuse, traumatic events, vascular injury and idiopathic origins. ONFH reduces the blood supply of the subchondral bone of the femoral head, which results in osteocyte death and the collapse of the articular surface. Once the collapse happens, the majority of patients inevitably need total hip replacement. But hip replacement surgery is not the best choice for each patient. Expect for the joint prosthesis factors, the patient's age and disease characteristics may result in several renovations. So more and more scholars suggest that we would better reserve the femoral head. Currently, such surgerys include osteotomy, core decompression, bone graft surgery, tantalum rods surgery, stem cell therapy, etc. The aim of the present review is to summarize the hip-reserving surgerys for ONFH by different mechanisms from a new perspective.

    Clinical application management of surgical ICU ventilator

    Objective:Discuss clinical application management of ventilator in surgical ICU. Method:Through the analysis of the structure and working principle of the ventilator and usage in surgical ICU . Clarify the problems existing in the clinical application of ventilator in surgical ICU . Results:Propose suggestions and measures in the clinical application of ventilator in surgical ICU, including the establishment of training system, regular maintenance and inspection, to strengthen the management of disinfection. Conclusion: Improve the clinical application ability and clinical application safety of ventilator in surgical ICU. Reduce the risk of infection.

    The treatment and nursing intervention of postoperative hypokalemia after extracorporeal circulation

    Objective To evaluate the efficacy and safety of rapid correction of postoperative early hypokalemia after extracorporeal circulation in heart disease using concentrated intravenous potassium infusions.Methods 63 patients with hypokalemia after extracorporeal circulation surgery were treated by potassium chloride infusions via central vein. Plasma potassium concentration,arterial blood gas analysis(ABG)and electrocardiographic were taken during therapy, Results No patient was died from hypokalemia. Conclusions It can rapidly and and safely correct hypokalemia in postoperative early hypokalemia after extracorporeal circulation using concentrated intravenous potassium infusions.
