    Relative factors analysis of early esophageal carcinoma risk of lymph node metastasis

    [Abstracts] Background Endoscopic technology provide a new choice for the therapy for early esophageal carcinoma, but it is Unable to complete lymph node cleaning . So it is definitely necessary to assess risk of lymph node metastasis before screening patients to have an operation or endoscopic therapy. Objective The aim of this study was to analysis risk factors for lymphatic metastasis in T1 esophageal squamous carcinoma and evaluate its risk. Methods Cases and pathological reports of 167 T1 esophageal squamous carcinoma were retrospectively analyzed, 133 patients(group N0) had no lymph node metastasis seen as control group and 34 patients(group N1) had lymph node metastasis seen as experimental group. Tumor specimens were reevaluated for overall submucosal layer thickness, tumor length, depth of tumor infiltration as well as lymphatic and vascular infiltration. Depth of submucosal tumor infiltration was divided in six layer (M1、M2、M3、SM1, SM2, SM3) and factors influencing lymphatic metastasis were assessed. Results Univariate logistic regression analysis showed depth of tumor infiltration, tumor length and tumor differentiation were risk factors influencing lymphatic metastasis formation. Variate logistic regression analysis showed depth of tumor infiltration(p=0.046), tumor length(p=0.046) and tumor differentiation(p=0.021) were independent risk factors influencing lymphatic metastasis formation(p<0.05). Conclusion Lymphatic metastasis risk of T1 esophageal squamous carcinoma is high and influencing factors is more, so esophagectomy should sequentially be the standard treatment for T1b patients prone to lymphatic metastasis.

    Features and risk factors analysis of Postoperative Pulmonary Complications of Esophageal Carcinoma

    Objectives:To analyze the features and risk factors of Postoperative Pulmonary Complications of Esophageal Carcinoma.Methods:A total of 295patients who received esophagectomy between March 2014 and May 2015 were included in the study. The patients were assigned into PPC(postoperative pulmonary complications)group and normal group, and postoperative factors were collected and analyzed between two groups, and the multivariate analysis (Logistic regression) were use to identify the risk factors of PPC. Conclusions:①No cases of death occurred in the two groups. The over incidence of PPC was 12.9%(38/295).②Univariate analysis revealed that there were significant differences in age,weight,smoking, preoperative pulmonary function, volume overload(all P<0.05). ③Three variables were found to be significantly related to the development of PPC by multivariate analysis: age(OR=8.71,P<0.05), smoking(OR=12.21 P<0.01), weight(OR=14.43,P<0.01). Results:PPC is a serious complication after esophagectomy. This study shows that age>70 years, smoking and emaciation are risk factors most likely associated with PPC development.

    Expression of p53, CK20 and C-erbB-2 in uroepithelium carcinoma of urinary bladder and their significance

    [Abstract] Objective To study the expression and significance of p53, CK20 and C-erbB-2 in uroepithelium carcinoma of bladder(BUC). Methods The expression levels of p53, CK20 and C-erbB-2 in 118 cases of BUC and 21 cases of benign lesion were detected with immunohistochermical EnVision method. Results The positive expression rates of p53 was 88.1%,CK20 were 26.3%, and C-erbB-2 were 45.8% in 118 cases of BUC. The expression of p53 in BUC was significantly increased associated with the benign lesions(P<0.01). The weakly positivity rate of p53 in low grade group was significantly more strong than in high grade group (P<0.01), but the strong positivity rate was significantly more lower than in high grade group (P<0.01). The positivity rate of CK20 in the benign lesions was lower than BUC, and the positivity rate of CK20 in low grade group was significantly more lower than in high grade group (P<0.01). There was significantly difference in expression of C-erbB-2 in BUC and the benign lesion(P<0.01). The positive expression rates of C-erbB-2 was higher and more strong in high grade group than in low grade group (P<0.01). The positive expression in low grade group always was weak or located in kytoplasm. Conclusion The overexpression of p53, CK20 and C-erbB-2 protein was observed in BUC, and their positive expressions were correlated with tumor grade, diagnosis and differential diagnosis.     Study of Correlation of vasculogenic mimicry and liver metastasis of colorectal cancer

    Objective Explore the relationship between vasculogenic mimicry and liver metastasis of colorectal cancer. Methods Analysis of vasculogenic mimicry formation in 115 colorectal carcinoma patients by immunohistochemical staining. Among the colorectal carcinoma patients, 25 were with liver metastases and 90 without liver metastases. Monitor cancer recurrence and liver metastases of these 115 patients by surveying for 2 years. Restults Among patients with liver metastases, 64.0% were VM positive. While only 26.7% were VM positive among patients without liver metastases. Recurrence and liver metastases of patients without liver metastases before operation were statistically significant with VM existence with the P values of 0.0003 and less than 0.0001, respectively. Conclusion VM was positively associated with recurrence and liver metastases of colorectal cancer.

    Investigate the relevant factors for nosocomial infections in patient with hypertensive intracerebral hemorrhage

    Objective To investigate the influencing factors for nosocomial infections in patient with hypertensive intracerebral hemorrhage and provide evidence for preventing and controlling nosocomial infections. Methods Retrospective analysis the relevant factors of nosocomial infections founded on the clinical data of 621 cases of patients with hypertensive intracerebral hemorrhage that come from our departments from 2011 to 2015.Results Of 621 patients,81 cases of them occurred nosocomial infections, the rate is 13.04%.The primary sites of infection are the lower respiratory tract and the urinary tract. The age of patient, location of cerebral hemorrhage, disturbance of consciousness, blood sugar levels and invasive procedures is association with the nosocomial infections. Conclusions The rate of nosocomial infections of patients with hypertensive intracerebral hemorrhage is greater higher. We must take targeted measures to prevent and combat nosocomial infections.

    Clinical effect comparison of Niti-Patellar Concentrator NT-PC and kirschner wire in treating aged comminuted patella fracture

    Abstract Objective: To analyze the clinical effect of Niti-Patellar Concentrator NT-PC and kirschner wire in treating aged comminuted patella fracture. Methods: 80 aged patients with comminuted patella fracture in our hospital from May 2010 to May 2015 were selected. They were numbered consecutively and the first 40 cases were control group and given kirschner wire. Other 40 cases were observation group and given Niti-Patellar Concentrator NT-PC. Compared the operative effect and complication occurrence rate of the two groups. Results: After treatment, clinical symptom approved, operation time, blood loss of the operation, wound healing time, hospital stays of observation group obviously shorter than control group, complication occurrence rate of observation group was 7.50%, and that of control group was 50.00%, the difference was statistically significant (P<0.05). Conclusion: The application of Niti-Patellar Concentrator NT-PC in treating aged comminuted patella fracture can shorter operation time and the occurrence of complication, the clinical effect is better.

    The effect analysis of tibiotalar Arthrodesis with inverted Philos plate and Medial screws by lateral incision

    Objective: To evaluate clinical efficacy of the lateral incision for tibial talocalcaneal Arthrodesis by using inverted Philos Plate and Medial Hollow screw. Methods: In between June 2012 to December 2014, lateral approach incision of ankle inversion Philos plate and the medial tibial subtalar joint with cannulated screws for the treatment of 12 cases of ankle joint lesion, post-operative follow up after 1 month and 3,6,12 months, for observation of wound healing, pain, subtalar Joint Fusion, fixation and ankle function. Results Patients were followed up after a mean follow-up time of 18.6 months. After 2 weeks of operation incision healed properly, no infection and the tissue necrosis; lateral side numb ness observed in one case, which disappeared after 3 months. No one complain about painful movement of ankle joint, on CT examination good fusion of ankle joint, and none of the internal fixators loose after 3 months of operation. After 12 months of operation, no one complained about painful movement ankle joint, good fusion and no any migration of the internal fixators. Ankle and lateral foot score average was77.5 according to AOFAS score. Conclusion: clinical efficacy of the lateral incision for tibial talocalcaneal Arthrodesis by using inverted Philos Plate and Medial Hollow screw has excellent clinical AOFAS score, which can meet the livelihood desire for relieving ankle joint pain. As well this is also effective on severe deformity correction of ankle joint.

    [Abstract] Objective: To explore the analysis of alprostadil combined with intracavitary therapy for the treatment of lower limb arteriosclerosis obliterans of efficacy and safety. Methods: our hospital in February 2012 - 2016 January 60 patients after lower extremity arteriosclerosis occlusion disease patients with lower extremity artery balloon dilatation and stent placed in operation, were randomly divided into two groups, than group carries on the conventional treatment, the observation group use of alprostadil, the change degree of the treatment were compared between the two groups before and after vascular ultrasound. Analysis therapeutic effects in the two groups. Results: after the treatment to observe group of foot dorsal artery blood flow velocity and ankle brachial index (ABI) to improve the situation was obviously better than that of the reference group, P < 0.05; observation group The total efficiency of the treatment is higher than that of the reference than the group, P < 0.05. Conclusion: alprostadil can significantly expand the patient's arterial stenosis in patients with, the establishment of collateral circulation process for the promotion and protection of, to improved blood circulation, relieve the clinical symptoms.

    Two cases of thymoma with spontaneous regression characteristics and literature review

    Abstract: Purpose To investigate clinicopathological features of thymoma with spontaneous regression including extensive hemorrhage, necrosis and cystic changes, so as to deepen the understanding of this entity type of thymoma. Methods clinicopathological features of two cases of thymomas with extensive hemorrhage, necrosis and cystic changes were analyzed and discussed combing related literature. Results The prognosis of two cases were both good with follow-up time 28 months and 6 months respectively, one case is a female patient, 61 years old with B2 thymoma , Masaoka stage III, and the other case is a male patient, 65 years old with B2 thymoma, Masaoka stage I. Conclusions This entity of thymoma with extensive hemorrhage, necrosis and cystic changes may have inert biological behavior, but the more clinic data and prolonging follow-up time were needed to evaluate the prognosis of these patients.

    Associating liver partition and portal vein ligation for staged hepatectomy (ALPPS): Breaks the barrier between resectable and unresectable.

    Liver failure is still the main issue for the post hepatectomy mortality. The main obstacle for hepatectomy is insufficient future liver remnant (FLR). Safety markers of hepatic resection are borderline liver function and sufficient volume of FLR. Associated liver partition and portal vein ligation for staged hepatectomy (ALPPS) is a new innovation to increase FLR to the desired volume permitting extended hepatectomy for patients with initially insufficient FLR. This article reviews the emergent and surgical aspect of ALPPS in comparison to traditional methods to increase FLR. Pathophysiology of liver hypertrophy following ALPPS and other methods are discussed in details. Outcome and prognosis are also reviewed through relevant literatures.

This paper makes a brief introduction about the new classification of lung tumour, and briefly introduces the main changes of the classification in 2015, and analyzes and discusses the problems that should be paid attention to in clinical practice.

    Objective To report 1 case of fetal death induced by amniotic band syndrome ,explore the amniotic band syndrome associated malformations associated factors and diagnosis.Methods Results In this case, the amniotic band syndrome is expressed as the amniotic band attached to the root of the umbilical cord.Although not lead to deformity of the appearance of the fetus,but causing abnormal umbilical cord, umbilical cord occlusion resulting in fetal death in utero, it is rare.This example stay after successful induction delivery of the dead fetus, repeated inspection to exclude other fetal malformations. Placenta, umbilical cord careful examination, repeatedly asked in detail about the history, because the patient refused further chromosome examination, the amniotic membrane with causes of this syndrome is not clear.Results Amniotic band syndrome is a rare clinical disease. the amniotic membrane with causes of this syndrome is not clear. Conclusion Amniotic band syndrome is a rare clinical disease. Amniotic band syndrome mainly for outer surface of tissue and organ defects; involvement of tissue and organ because of its occurrence in the critical period,because of the amniotic sac and amniotic membrane rupture with adhesions interference and teratogenicity; the severity of the deformity and range depends on the amnion rupture time and site.we have to strengthen the understanding of this disease through rare clinical cases, and accumulated valuable experience.
