    Relationship between circulating tumor cells and clinical pathologic stages in patients with colon cancer

    Objective To understand the relationship between circulating tumor cells (CTCs) and clinical pathologic stages in patients with colon cancer.Methods CTCs counts in peripheral blood of untreated patients with colon cancer were detected by CellSearch technique.The relationship between CTCs and clinical pathologic stages of colon cancer was analyzed.Results CTCs in peripheral blood were detected in 40.6% (13/32) untreated cases of colon cancer.Significant relationships of positive CTCs were identified with colon cancer infiltration, TNM stage, and blood vessel infiltration (P<0.05), but not with age, sex, tumor size, and nerve infiltration.The Ⅲ+Ⅳ TNM stage patients had a much higher positive rate of CTCs than the Ⅰ+Ⅱ TNM stage patients (P<0.05).Conclusion CTCs can be detected in the peripheral blood of colon cancer patients, indicating higher clinicopathological stages.

    Clinical efficacy of nipple-sparing mastectomy and immediate breast reconstruction for early breast cancer patients

    Objective To investigate the clinical efficacy, complications, and cosmetic results of reserving the nipple-areola complex (NAC) and immediate breast reconstruction for early breast cancer patients.Methods A total of 58 cases of breast cancer (0 -Ⅱa stage) were retrospectively reviewed in the Department of Surgical Oncology, Fujian Provincial Hospital from June 2010 to June 2014.Of these patients,32 cases underwent nipple-areola complex sparing mastectomy (NSM) and immediate breast reconstruction(observation group), and the other 26 cases underwent modified radical mastectomy and immediate reconstruction (control group).The reconstruction was performed using a prosthetic implant, covered with dorsal latissimus free flap. The local recurrences, distant metastasis, complications and cosmetic outcomes were compared.Results The follow-up time ranged from 48 to 60 months with a mean of 52.36±3.51 months. No local recurrence was found in the two groups. One case of distant metastasis occurred in both the observation group and control group, and there was no significant difference (P>0.05). The postoperative complications were found in 11 cases of the observation group and in 7 cases of the control group, respectively, but there was no significant difference (P>0.05). For the observation group,the number of cases rated as excellent, good, not bad, and bad levels of cosmetic outcomes were 26, 5, and 0, respectively, and for the control group, the number was 6, 14, 6, and 0, respectively;the cosmetic outcomes of the observation group were significantly better than those of control group (P<0.05).Conclusion Nipple-areola complex sparing mastectomy and immediate breast reconstruction contribute to achieving better aesthetic and functional results, also providing a cosmetic and quality-of-life benefit for patients selected by strict preoperative selection criteria and following strict operative procedure.

    Clinical application of injury control surgical technique tosevere abdominal trauma emergency rescue

    Objective To analyze the clinical effect of injury control surgical technique to severe abdominal trauma emergency rescue.Methods A total of 98 patients with severe abdominal trauma admitted from March 2014 to November 2016 were divided into an observation group and a control group, with 49 cases in each group.The control group was given traditional emergency rescue technique,and the observation group received injury control surgical technique.APACHE II scores, body temperature, oxygen saturation, pH,successful rescue rate, and complications were recorded and compared before and after recovery between the two groups.Results The successful rescue rate of the observation group was higher than that of the control group (95.92% vs 83.67%, P<0.05).The complication rate of the observation group was lower than that of the control group (12.24% vs.28.57%, P<0.05).Before recovery, no differences in APACHE II score, oxygen saturation, pH, and body temperature were observed between the two groups. After recovery, body temperature, oxygen saturation, pH, and APACHE II score of the observation group were superior to those of the control group (P<0.05).Conclusion Injury control surgical technique for severe abdominal trauma emergency rescue has a better effect with less complications. Thus, it is worthy of clinical promotion.

    Comparison of clinical applications of single-incision and multiple-incision thoracoscopic lobectomy

    Objective To investigate the clinical effects of single-incision and multiple-incision thoracoscopic lobectomy, and illustrate the significance and value of single-incision thoracoscopic lobectomy in clinical application.Methods A total of 298 patients with thoracoscopic lobectomy were selected from January 2013 to December 2015 and divided into single-incision (n=148) and multiple-incision (n=150) thoracoscopic lobectomy groups according to the operation methods they received.The operative effects between the two groups were compared.Results The amount of blood loss, dosage of hydrochloric acid, and length of hospital stay of the single-incision thoracoscopic lobectomy group were significantly less than those of the multiple-incision thoracoscopic lobectomy group (P<0.01). There were no differences in operation time, number of lymph node removed, and complication rate between the two groups (P>0.05). The VAS scores on the 3rd and 7th days of the single-incision thoracoscopic lobectomy group were all significantly lower than those of the multiple-incision thoracoscopic lobectomy group at the same time points (P<0.01). No statistical differences in the indicators related to quality of life were found between the two groups before operation (P>0.05). The function, emotion, and total scores of the single-incision thoracoscopic lobectomy group were significantly higher than those of the multiple-incision thoracoscopic lobectomy group after 3 months of operation (P<0.01).Conclusion Compared with multiple-incision thoracoscopic lobectomy, single-incision thoracoscopic lobectomy can effectively reduce surgical trauma, promote postoperative recovery, and improve the short-term quality of life of patients.

    Significance of fPSA/tPSA, PSAD, and PSATZ in diagnosis of prostate cancer

    Objective To explore the diagnostic significance of ratio of free prostate specific antigen to total prostate (fPSA/tPSA), prostate specific antigen density (PSAD), and prostate specific antigen transition zone density (PSATZ) in serum prostate specific antigen (PSA) gray area for prostate cancer (PCa).Methods A total of 173 cases with PCa and benign prostatic hyperplasia (BPH) whose tPSA levels were 4-10 ng/mL were selected in the study. Serum fPSA and tPSA were detected, and fPSA/tPSA ratio was calculated. Prostate volume was determined by transtectal ultrasound. PSAD and PSATZ were calculated.The diagnostic significance of fPSA/tPSA, PSAD, and PSATZ in PSA gray area for PCa were analyzed using Python 3.5 software.Results The PCa patients’ and the BPH patients’ fPSA/tPSA ratios were 0.17±0.072 and 0.2±0.085 respectively.Their PSADs were (0.25±0.11) μg/L and (0.14±0.054) μg/L respectively. Their PSADTZs were (0.53±0.31) μg/L and (0.25±0.14) μg/L respectively. The two groups showed significant differences in the above three indicators (P<0.05). The sensitivity of fPSA/tPSA and PSAD for PCa reached 79%.Conclusion Both PSAD and PSATZ in PSA gray area can increase sensitivity in the diagnosis of PCa. Moreover, fPSA/tPSA, PSAD, and PSADTZ are of great value for the diagnosis of PCa.

    Treatment of Mallet fractures using hook plate combined with K-wire technique

    Objective To introduce the surgical technique and clinical outcome of treatment of Mallet fractures using hook plate combined with K-wire.Method From January 2014 to January 2016, 11 cases of Mallet fractures were treated with hook plate combined with K-wire. A dorsal S or L shaped incision was made at the distal interphalangeal (DIP) joint to expose the fracture site. Under direct visualization the fracture was reduced and fixed with mini hook plate, then use a K-wire to fix the DIP joint. The K-wire was extracted and DIP joint functional exercise was strengthened 4 to 6 weeks after operation.Results All the 11 cases were followed-up for 6 to 24 months with an average of (15±6) months. All the fractures healed successfully with an average of (5.5±1.5)weeks. Based on Crawford’s evaluation criteria of DIP joints, the results were excellent for 5 cases, good for 4 cases, and fair for 2 cases. Raised ridge of nail was seen in 1 case.There were no arthritis, infection, intemal fixation failure, and other complications.Conclusion The application of hook plate combined with K-wire technique in the treatment of Mallet fractures not only retains the advantages of plate treatment, but also increases the stability of fracture site, showing satisfactory treatment effect.

    Osteogenic activity induced by complex of bioactive glass ceramics,bone morphogenetic protein, and biomedical fibrin glue

    Objective To investigate the osteogenic activity induced by the complex of bioactive glass ceramics (BGC), bone morphogenetic protein (BMP), and biomedical fibrin glue (BFG).Methods Twenty SD rats were randomly divided into A, B, and C groups and each group as further divided into two subgroups. The complex of BGC, BMP, and BFG as used in subgroup 1, and ceramic artificial bone as used in subgroup 2. The transplantation effect was observed after 4 weeks.Results The implantation efficiency rate (70% vs. 60%), tissue growth efficiency rate (60% vs. 40%), and activity of alkaline phosphatase (0.441±0.082 vs.0.234±0.022) of the subgroup 1 were significantly higher than those of the subgroup 2 (P<0.05).Conclusion The complex of BGC, BMP, and BFG could ossify implants.

    Clinical analysis on internal fixation with titanium embracing fixator for multiple rib fractures

    Objective To evaluate the effect of internal fixation with titanium embracing fixator on multiple rib fractures.Methods Clinical data of 188 patients with multiple rib fractures treated with internal fixation with titanium embracing fixator (operation group, n=52) or conventional conservative treatment (non-operation group, n=136) between January 2011 and January 2016 were retrospectively analyzed and compared.Results The duration of pain was significantly reduced in the operation group compared with the non-operation group (P<0.05). The incidence rates of pulmonary infection and atelectasis in the operation group was lower than those in the non-operation group (P<0.05). The patients in the operation group recovered more quickly than the non-operation group.Conclusion Applying internal fixation with titanium embracing fixator to the treatment of multiple rib fractures can relieve pain, reduce pulmonary complications, and shorten recovery time.

    Clinical effects of two surgery methods on stiff knee replacement

    Objective To compare the clinical effects of different stiff knee replacement surgery methods.Methods From January 2009 to March 2015, 80 cases (100 knees) of ankylosis were randomized into two groups according to the knee replacement surgeries they received: traditional knee replacement and knee replacement with tibial tubercle osteotomy. X-ray images, the rang of motion (ROM), knee society score (KSS), and functional scores were compared between the two groups after operation and were followed up for 3 months.Results Both methods had significant effects on stiff knee replacement. Compared with traditional knee replacement, ROM, KSS, and functional scores after knee replacement with tibial tuberosity osteotomy were higher (P<0.05). Postoperative complications and infection rates were not different between the two groups (P>0.05).Conclusion Knee replacement combined with tibial tubercle osteotomy has a better effect than conventional knee replacement, which is worthy of clinical application.

    Objective To assess the clinical value of minimally invasive stabilization system in the treatment of complicated fracture of distal femur.Methods Sixty-two patients with complicated fracture of distal femur treated from February 2015 to February 2017 were selected in the study. The subjects were randomly divided into a test group and a control group according to random number table, with 31 cases in each group. The test group adopted an approach using minimally invasive stabilization system; while the control group used internal fixation with anatomic plate. Operation time, intraoperative bleeding volume, and bone healing conditions were compared between the two groups.Results The test group showed less operation time, intraoperative bleeding volume,and bone healing time than the control group (P<0.05). The excellent and good rate of knee joint function recovery of the test group (90.32%) was significantly higher than that of the control group (67.74%) (P<0.05).Conclusion The minimally invasive stabilization system has an obvious effect on complicated fracture of distal femur, showing shortened operation time and less body injury, which is worthy of clinical application.

    Effect of hyperbaric oxygen combined with electroacupuncture at Siguan acupoint on early cognitive impairment in stroke patients

    Objective To observe the effects of hyperbaric oxygen combined with electroacupuncture at Siguan acupoint on early cognitive impairment in stroke patients.Methods Ninety stroke patients with mild cognitive impairment were randomly divided into Siguan acupuncture group, hyperbaric oxygen group,and hyperbaric oxygen combined with Siguan acupuncture group. The therapeutic effects were observed before and after treatment.Results After two courses of treatment, the Siguan acupuncture group and the hyperbaric oxygen group showed improved MMSE score (P<0.05). After one course of treatment, the hyperbaric oxygen combined with Siguan acupuncture group showed improved MMSE score (P<0.05). After two courses of treatment, there was a significant difference in the MMSE score among the three groups (P<0.05).Conclusion Hyperbaric oxygen combined with acupuncture at Siguan acupoint can significantly improve cognitive impairment in stroke patients and is superior to acupuncture or hyperbaric oxygen therapy alone.

    Effects of different surgical treatments for uterine myoma on ovarian function

    Objective To evaluate the short-term and long-term effects of different surgical methods for uterine myoma on ovarian function by detecting the serum levels of AMH and postoperative modified Kupperman score.Methods From January 2014 to July 2015, 120 uterine fibroids patients underwent surgery, including uterine myomectomy group (group A, n=40), hysterectomy group (group B, n=40), and hysterectomy plus bilateral tubal resection group (group C, n=40). Serum AMH levels were detected and compared among the three groups before 2 days of surgery and after 1, 6, and 12 months of surgery, and modified Kupperman scores at corresponding time points were also detected to assess the severity of perimenopausal symptoms.Results For group B and group C, AMH levels were significantly lower at postoperative 6 and 12 months than preoperative 2 days (P<0.05). Compared with group A, group B and group C showed significantly lower levels of AMH at postoperative 6 and 12 months (P<0.05). For group B and group C, modified Kupperman score did not change at postoperative 1 month (P>0.05), but increased significantly at postoperative 6 and 12 months (P<0.05) compared with preoperative 2 days. Compared with group A, group B and group C showed higher modified Kupperman scores at postoperative 6 and 12 months (P<0.01).Conclusion Myomectomy (control group) has little effects on ovarian function, but its clinical effect is comparable to subtotal hysterectomy. The patients receiving hysterectomy or hysterectomy with bilateral tubal resection result in decreased ovarian function at postoperative 6 and 12 months. Therefore, myomectomy is a preferred treatment for uterine myoma without affecting ovarian function.

    Clinical effects of CO2 laser on early glottic cancer

    Objective To explore the clinical effects of CO2 laser for early glottic cancer.Methods The clinical data of 40 cases with early glottic cancer from May 2012 to May 2017 were selected, including 24 cases of T1a lesion, 9 cases of T1b lesion, 7 cases of T2 lesion, 34 cases without involvement of glottis joint, and 6 cases with involvement of glottis joint. After CO2 laser, the cases were followed up for 6-72 months.Results In the 40 cases, 5 cases (12.5%) recurred after surgery. The recurrence rates for T1a, T1b, and T2 lesion patients were 4.2%(1/24), 11.1%(1/9), and 42.9%(3/7), respectively (χ2=7.4376, P=0.0243). For the patients without involvement of glottis joint, the recurrence rate was 11.8% (4/34).For the patients with involvement of glottis joint, the recurrence rate was 16.7% (1/6) (χ2=0.1120, P=0.737 8). In the 40 cases, 39 (97.5%) cases survived and 1 died (2.5%).Conclusion CO2 laser can preserve voice function of larynx with good results for early glottic cancer, thus deserving promotion.

    FBN1 gene and ADAMTSL4 gene mutations in three lens subluxation families

    Objective To screen the genetic mutations in FBN1 and ADAMTSL4 genes from three families who had congenital lens subluxation by targeted gene capture with high-throughput and Sanger sequencing techniques.Methods Peripheral blood samples were collected from 14 lens subluxation patients and 3 healthy members of three families and genomic DNA was isolated. FBN1 was selected by a gene capture strategy with 65 exon capture probes. The enrichment libraries were sequenced on HiSeq2000 sequencer to determine the mutation frequency in FBN1. Probable mutations were verified by Sanger sequencing method. At the same time, ADAMTSL4 gene from three probands was also analyzed by Sanger method.Results Three different hybrid missense mutations were found in FBN1 gene from the three families. Four single nucleotide polymorphism changes were found in ADAMTSL4 gene.Conclusion FBN1 gene is the pathogenic gene that cause congenital lens subluxation in the selected three families. FBN1 genetic mutations are the primary causes leading to the clinical signs of lens subluxation.

    Effect of early fluid management on survival of patients with septic shock

    Objective To investigate the effects of different early fluid management approaches on the death risk of patients with septic shock.Methods Basic information, SOFA score, and daily intake and output volume of 119 patients with septic shock were collected. Correlation between liquid volume and outcome of patients with septic shock was analysed based on the liquid intake and output volume.Results The daily fluid balance in the first two days of the dead group was significantly higher than that of the survival group (P=0.031). The survival rate was significantly lower in the patients in the third quartile of 2-day cumulative fluid balance versus those in the first quartile (P=0.037). After 2 days of shock, the death risk in the high SOFA score group (>5 points) was higher than that in the low SOFA score group (<5 points) (P<0.001). The death risk was increased to 2.957 times in the patients in the third quartile of 2-day cumulative fluid balance (P=0.012).Conclusion The septic shock patients’ prognosis is related to the volume of early fluid input. The patients with a large volume of fluid intake showed poor prognosis. The SOFA score can reflect the severity of septic shock patients.

    Clinical observation of laparoscopic-assisted Parks surgery for low rectal carcinoma

    Objective To observe the clinical effect of laparoscopic-assisted Parks surgery on low rectal carcinoma.Methods Sixty-three patients with low rectal carcinoma treated from January 2013 to January 2017 were divided into two groups according to different therapies. The observation group (n=47) received laparoscopic-assisted Parks surgery, and the control group (n=47) received laparotomy. The incidence rate of postoperative complications, defecation function, operation time, bleeding volume, hospitalization time, and anal exsufflation time were recorded.Results The operation time, bleeding volume, anal exsufflation time, and hospitalization time of the observation group were significantly less than those of the control group (all Ps<0.05).The incidence rate of postoperative complications of the observation group (6.38%, 3 cases) was significantly lower than that of the control group (17.02%, 8 cases). The good recovery rate of defecation function of the observation group (55.32%, 26 cases) was significantly higher than that of the control group (31.91%, 15 cases), while the poor recovery rate of defecation function (8.51%, 4 cases) was significantly lower than that of the control group (21.28%, 10 cases).Conclusion Laparoscopic-assisted Parks surgery is better than laparotomy for the treatment of rectal carcinoma,deserving clinical promotion.

    Exploration of pathogenesis and clinical treatment for sacroiliac joint pain after lumber fusion

    Sacroiliac joint pain is one of the most important types of low back pain after lumber fusion. At present, the diagnostic criteria of sacroiliac joint disease are not unified. Its etiology is complicated,and its pathogenesis is not clear. It is difficult to clearly display the abnormal changes of sacroiliac joints by imageology examination,diagnostic basis is not sufficient, and the rates of missed diagnosis and misdiagnosis are very high. Etiology, path-ogenesis, and clinical treatment of sacroiliac joint pain after lumber fusion were systematically reviewed in this article, hoping to provide a more reliable basis for the early prevention, early diagnosis, and clinical treatment of sacroiliac joint pain after lumber fusion.

    Analysis on factors affecting urinary incontinence in postpartum women

    Urinary incontinence is a common chronic disease in gynecology and has gained increasing attention with the improvement of women’s social status. According to relevant statistics, the incidence rate of postpartum urinary incontinence is 36.4%. Studies have shown that the risk of urinary incontinence is 4 times higher for those who had urinary incontinene during pregnancy and puerperium than for normal women. In this paper, the causes of postpartum urinary incontinence were reviewed, and the factors of postpartum urinary incontinence were summarized and analyzed, aiming to guide future prenatal and postnatal education and rehabilitation and to consequently prevent postpartum urinary incontinence.

    This review summarized diagnosis, etiology, risk factors, and harm of the elderly female stress urinary incontinence, expounded the types of stress urinary incontinence among elderly women, treated by non-surgical treatment, and focused on research hot spots in recent years, including lifestyle intervention, voiding habits training, pelvic floor reconstruction rehabilitation, biofeedback and electrical stimulation therapy, and pelvic floor rehabilitation combined with Chinese medicine treatment. In particular, physical therapy and internet plus nursing benefit treatment of urinary incontinence are new hot spots recently. The paper also analyzed the weaknesses and limitations of pelvic floor rehabilitation, the most effective method at present. Medical personnel are required to formulate a practical, simple, and effective treatment.

    Effect of CICARE communication mode on improvingsatisfaction of patients with spinal cord injury

    Objective To investigate the effect of “connet, introduce,communicate, ask, and respond” (CICARE) communication mode on the satisfaction of patients with spinal cord injury.Methods We selected 30 cases of inpatients who were treated with traditional nursing mode (control group) and 30 cases who were treated with additional CICARE communication mode (observation group) in 2016 to conduct a patient satisfaction survey. The satisfaction with nursing care was compared between the two groups.Results The satisfaction level of the patients with spinal cord injury after treatment with CICARE communication mode was significantly higher than that of the control group (P<0.05).Conclusion CICARE mode can improve the quality of life of patients, both nurses’ and patients’ satisfaction, and nurses’ communication skills, psychological nursing ability, and management ability.
