    The incidence of ischemic cerebrovascular diseases is stiy high in current society.Both high morbidity and mortality have become an important role in negatively impacting the quality of life in humans.How to prevent and treat ischemic cerebrovascular diseases is an important medical and social problam.Some key points will be discussed here with special regard to the application of extra-intracranial vascular reconstruction on the treatment of ischemic cerebrovascular diseases.

    Over the past several years,state of the art technologies previously used in other specialties,as well as new technologies,have been introduced and developed in anesthesiology and patient monitoring.This review described the applications of digital video technology (fiber optic bronchoscope,digital bronchoscope,and video-laryngoscope) and the ultrasound technology in anesthesiology.It also described the integrated anesthesia information system and anesthesia depth monitoring,new drug delivery concept termeel as target-controlled infusion and its delivery system.These new technologies were developed with the common goals for improving safety and effectiveness in anesthesia and perioperative medicine.

    Mastalgia is one of the common complains in the breast diseases of the fertile age women.The pain can be categoried into two types,periodicity pain and non-periodicity pain.As moderate and severe pain on the patients’ daily life,such pain should be cured with medicine.Timely confirming the pathgeon and categories of the mastalgia would take good effects on the therapy of the mastalgia and prevention of the breast cancer.

    Cognitive impairment and changes of neuronal plasticity in rats of    chronic cerebral hypoperfusion

    Objective A new cerebral arteriovenous malformations (AVMs) rat model was developed to study the effect of chronic cerebral hypoperfusion on cognitive function and neuronal plasticity in rats.Methods Aged-matched animals comprised a control group.Three months after surgery,Morris water waze test was performed to evaluate the cognitive function in rats.Neuronal plasticity was assessed by measuring the protein expression of MAP-2,GAP-43 and synaptophysin in the hippocampal regions of rats with immunohistochemistry and western blotting.Results The average time of escape latency was significantly longer in the model rats than that in the control rats,and both the time spent in the platform quadrant and the frequency of original platform crossing during space probe trials were less than those in the control animals.The expression levels of MAP-2 and synaptophysin protein in hippocampal areas in the model rats were less than those in the control rats.However,there was no difference on the GAP-43 expression between the two groups.Conclusion These data suggested that chronic cerebral hypoperfusion associated with AVMs could lead to cognitive impairment in rats,which may be partially explained by reduced expression of MAP-2 and synaptophysin at the protein level in the hippocampal area.

    Exploring the operational time and application value of intracranialpressure monitoring in treatment of patients with bifrontal lobes of brain contusion and laceration

    Objective To explore the operation time and application value of intracranial pressure monitoring in treatment of patients with bifrontal lobe of brain contusion and laceration.Methods Clinical data of 66 patients with bifrontal lobe of brain contusion and laceration from 2005 to 2010 were studied retrospectively.66 patients were underwent bifrontal decompressive craniectomy.35 cases treated by bifrontal decompressive craniectomy combined with intracranialpressure monitoring were included in monitoring group.31 cases treated only by bifrontal decompressive craniectomy were included in control group.The clinical efficacy of both two groups were compared 12 months later.The data were retrospectively analyzed in order to summary the clinical characteristics,operational time,application value of intracranial pressure monitoring and prognosis.Results 8 cases got emergence operation,58 cases were operated during the medical treatment because their situation developed worse gradually.All of the patients were followed up for twelve months.According to GOS assessment,there are 3 cases died,1 case with persistent vegetative state,2 cases with severe disability,4 cases with moderate disability,25 cases got good recovery in monitoring group.There are 5 cases of dearh,2 case with persistent vegetative state,2 cases with severe disability,6 cases with moderate disability,16 cases got good recovery in control group.The favorable prognosis rate of monitoring group and control group was 71.43% and 51.61%(P<0.05).Conclusion The situation of many patients with bifrontal lobes brain contusion and laceration will become worse and worse gradually.The doctors should observe the patients carefully and operate on them in time.The bifrontal decompressive craniectomy is an effective method for the surgical management.The intracranial pressure monitoring can dynamically observe ICP,providing the objective and accurate evidence,and improve the prognosis of patients.

    Superior sagittal sinus metastasis from nasopharyngeal carcinoma:a case report and review of literature

    Nasopharyngeal carcinoma (NPC) is one of the most common malignancies in the head and neck,especially among Chinese population.Although NPC has a propensity to develop distant metastases at a high rate including bones,lungs,liver and distant lymph nodes,superior sagittal sinus metastases from NPC are very rare.We report a case of such a disease in a 54-year-old man with NPC who presented with heahache,vomiting and status epilepticus 24 months after finishing radiotherapy and chemotherapy.MRI showed the presence of a occupying lesion in right frontal lobe associated with superior sagittal sinus,which was initially misdiagnosed as a parasagittal meningioma.The patient received surgical treatment via craniotomy.Histologic and immunohistochemical analyses confirmed that it was original nasopharyngeal.After resection of the intracranial tumors,no signs of local or distant recurrence was noted during the 6 months follow up.Although surgical treatment plays a limited role in metastatic NPC,there is rare cases of intracranial sinus metastasis with a reasonable outcome after resections.

    Use of a mouth-facial supportor to facilitate mask ventilation in toothless elderly patients

    Edentulism and tooth loss are major causes of mask ventilation leakage (MVL).There is no effective method to prevent MVL in edentulous elderly patients at present.We designed a mouth-facial supportor in an attempt to minimize MVL due to the edentulism,and evaluated the efficacy of this device during mask ventilation in toothless elderly patients in this study.186 patients were enralled for elective surgeries with general anesthesia into the study.Among them,116 patients were toothless and randomly assigned to either Group A or Group B.The mouth-facial supportor was used for patients in Group A,whereas no device was used for those in Group B.The remaining 70 patients with normal dentition were assigned to Group C.Mask ventilation was performed after induction of anesthesia.The mask was held by a male anesthesiologist with over 10-year experience with single hand.Actual expiratory tidal volume,oxygen saturation (SpO2),blood pressure and heart rate were monitored and recorded.MVL in Group A and Group C were significantly lower than that in Group B (P<0.05).The success rates of mask ventilation in Group A,Group B and Group C were 83.3%,17.9% and 75.7% respectively.No difference was detected in the success rates between Group A and Group C;however,it was significantly higher than that in Group B (P<0.05).The result showed that our designed mouth-facial supportor raised the success rate of mask ventilation in toothless patients to 83.3%,comparable to that in toothed patients.

    The value of combination of MRCP and CA19-9 in clinical diagnosis of surgical obstructive jaundice

    Objective To evaluate the value of MRCP combined with CA19-9 in diagnosis of surgical obstructive jaundice.Methods The clinical data of 94 patients with surgical obstructive jaundice were analyzed.Results The correct diagnostic rates of each patient in finding causes of obstructive jaundice,CA19-9 were 76.3% and 30.4%,MRCP 83.8% and 91.1%,CA19-9 combined with MRCP were 94.7% and 82.9%.Conclusion MRCP and CA19-9 have their own advantages and disadvantages in diagnosing surgical obstructive jaundice.The combination can significant raise the accuracy of diagnosis.As complementary to each other,MRCP and CA19-9 together can provide reliable etiologic diagnosis for suspicious obstructive jaundice.

    Chronic cerebral hypoperfusion induced degeneration of nerve cells is one of the important factors in low social ability in patients with cognitive dysfunction,and is closely linked to pathology of neurodegenerative and cerebrovascular disease.Domestic and foreign researches on Endocannabinoid toxin (ECS) including G protein-coupled receptor CB1,CB2,AEA,2-AG endogenous ligand and its endogenous ligand deactivation system AEA hydrolase FAAH have shown that ECS has a neuroprotective effect of endogenous physiological regulation mediated in mammalian central nervous system,which is mainly manifested in the receptor level,deactivation system,immune responses and synaptic plasticity and so on.This article is to review the endocannabinoid system (ECS) effects on cognitive impairment in patients with chronic cerebral hypoperfusion.

    Stimulated by the 2012 Nobel Prize in Physiology or Medicine awarded for Shinya Yamanaka and Sir John Gurdon,there is an increasing interest in the induced pluripotent stem (iPS) cells and reprograming technologies in medical science.While iPS cells are expected to lead a new era providing enormous opportunities in biomedical sciences in terms of cell therapies and regenerative medicine,safety-related concerns for iPS cell-based cell therapy should be resolved prior to the clinical application of iPS cells.In this review,the pre-clinical investigations of cell therapy for spinal cord injury (SCI) using neural stem progenitor cells derived from iPS cells,and their safety issues in vivo,are outlined.We also discussed the strategy for the first human trails of iPS cell-based cell therapy for SCI patients.

    The breaking of ankle and or subtalar joint would lead to pain and functional limitation of the foot.The tibiotalocalcaneal arthrodesis(TTCA) refers to the tibiotalar joint and subtalar joint,simultaneously with the joint repair and integration,easing the pain of the foot and ankle to maintain stability.There are multiple materials with tibiotalocalcaneal arthrodesis,such as screws,external fixation,intramedullary nail and blade plate.It is important that suitable method of this operation should be selected. It is the objective of arthrodesis development to achieve a higher satisfaction rate for patients by adopting the simple technology,keeping the outstanding stability,reaching the higher healing rate and reducing the soft tissue damage and complications to maximal extent.

    Osteosarcoma is a kind of common orthopedic malignancies with poor prognosis.With rapid development of new techniques in recent years,the result of osteosarcoma is getting better and better.However,there is still no thorough treatment for this disease.With the development of molecular biology,more and more attention is focusing on the role of various signaling pathways in osteosarcoma.One of the signaling pathways is TWEAK/Fn14.It may play an important role in the formation of osteosarcoma.Research progress of the potential relationship between TWEAK/Fn14 signaling pathway and osteosarcoma will be elaborated and discussed.

    Discussion on the integration of resources and coordination of development in medical periodicals and journals of colleges in Shanghai

    Objective To discuss the ways and means of managing the medical periodicals in Shanghai with the purpose for sharing the resouces of information and data. Methods Information of the medical periodicals and journals of colleges in Shanghai during Nov 2009 to Jul 2011 was collected,by the questionnaire survey with 87 questions to editorial department of 45 medical journals performed online,holding the symposium and seminar,inspection and visiting. Results The returned questionnaire information of 42 journals showed: the rate of survey was 93.33%,including 34 Journals of Collages and 9 journals of Shanghai institutes for biological science of China Academy of science. The impact factors,degree of internationalization,and the degree of digital establishment of Shanghai institutes for biological science were higher than that of college medical journals(P<0.005). Conclusions Breaking the existing manage system gradually,following the collectivization and cooperative model,integrating the resources of college medical journals,improving management and balancing development are trends of journal development in line with marketing economics.
