    Effect of laparoscopic surgery on immune function and surgical complications in patients with colorectal cancer

    Objective To assess the effect of laparoscopic surgery in patients with colorectal cancer.Methods A total of 86 patients with colorectal cancer admitted from May 2017 to May 2018 were selected,and divided into two groups by using random number table,with 43 cases in each group.The control group was given open surgery,while the observation group was treated with laparoscopic surgery.Immune function and postoperative complications were compared between the two groups.Results After operation,the levels of immune function indices of the two groups showed desceasing trends,but the levels of the observation group showed smaller fluctuations than those of control group,and the incidence rate of complications in the observation group was also lower than that in the control group (P<0.05).Conclusion Laparoscopic surgery has advantages such as minimal trauma,low risk,and less complications,effectively reducing the influence on immune function of patients with colorectal cancer and improving their physical and mental health.

    Effect of laparoscopic radical gastrectomy combined with hyperthermic infusion chemotherapy on advanced gastric cancer

    Objective To analyze the clinical value of laparoscopic radical gastrectomy combined with hyperthermic infusion chemotherapy for advanced gastric cancer.Methods Eighty patients with advanced gastric cancer were selected from October 2017 to October 2018 in our hospital and divided into experimental group and control group with 40 cases in each group.The control group was treated with laparoscopic radical gastrectomy.The patients in the experimental group were treated with laparoscopic radical gastrectomy combined with hyperthermic infusion chemotherapy.The recovery time of gastrointestinal function,clinical treatment effect,postoperative hospitalization time,and incidence of complications were compared between the two groups.Results The total incidence rate of complications in the experimental group (5%) was less than that in the control group (P<0.05),so were the hospitalization time (P<0.05) and the recovery time of gastrointestinal function.The total effective rate of the experimental group (97.5%) was higher than that of the control group (75%,P<0.05).Conclusion For patients with advanced gastric cancer,laparoscopic radical gastrectomy combined with hyperthermic chemotherapy is effective by improving gastrointestinal function,shortening postoperative hospital stay,and lowering clinical complications.

    Analysis of factors associated with complications after colorectal cancer surgery

    Objective To study the factors associated with complications after surgery in patients with colorectal cancer.Methods A total of 133 patients with colorectal cancer were divided into complication group (n=31) and non-complication group (n=102) according to whether they had complications within 30 d after surgery.Relevant factors of complications in the two groups were analyzed.Results There were statistically significant differences between the two groups in age,medical complications,time of surgery,and neoplasm staging (P<0.05).The multiple logistic regression model indicated that medical complications,time of surgery,and neoplasm staging were the independent factors influencing the occurrence of surgical complications in patients with colorectal cancer.Conclusion The high incidence rate of surgical complications in patients with colorectal cancer indicates that it is very important to control related basic diseases before operation,improve operative skills so as to shorten operative time as much as possible.

    Enhanced recovery after surgery improves nutrition and immune function of patients with colorectal cancer

    Objective To observe the changes of nutrition and immune function indices of patients with colorectal cancer using enhanced recovery after surgery.Methods A total of 64 patients with colorectal cancer admitted from March 2016 to March 2018 were divided into control group (n=33) and observation group (n=31).The patients in the observation group received enhanced recovery after surgery; the patients in the control group received conventional treatment.The nutrition indices (ALB,PA,and SF) and immune indices (IgA,IgG,and IgM) of the two groups before surgery and 1 day,3 days,and 7 days after surgery were detected.Results In the observation group,except for a lower level 1 day after surgery (P<0.05),no differences in PA level were observed between 3 days or 7 days after surgery and before surgery.In the control group,the PA,ALB,and SF 3 days after surgery had statistical differences compared with before surgery(P<0.05) and returned to the preoperative level 7 days after surgery.The changes of ALB,PA,and SF levels of the observation group were smaller than those of the control group (P<0.05).The postoperative immune function indices of both groups changed,and the levels 3 days after surgery were lower than before (P<0.05),and returned to the preoperative level 7 days after surgery.The changes of IgA,IgG,and IgM levels of the observation group were smaller than those of the control group as well (P<0.05).Conclusion Enhanced recovery after surgery could relieve the nutritional status and immune function of patients with colorectal cancer.

    Clinical efficacy of comprehensive therapy in the treatment of early inflammatory intestinal obstruction after acute abdominal surgery

    Objective To evaluate the clinical efficacy of comprehensive therapy in the treatment of early inflammatory intestinal obstruction after acute abdominal surgery.Methods Eighty-two patients with early inflammatory intestinal obstruction who were treated in our hospital from February 2016 to June 2018 were randomly divided into control group and observation group,41 cases in each group.The patients in the control group were treated with traditional surgery,while the patients in the observation group were treated with comprehensive therapy.The clinical effects and clinical indicators (anal exhaust time,abdominal distension disappearance time,and bowel sounds recovery time) were compared between the two groups.Results After treatment,the procalcitonin(PCT) and c-reactive protein(CRP) levels in the observation group were significantly higher than those in the control group (P<0.05); the anal exhaust time,abdominal distension disappearance time,and bowel sounds recovery time in the observation group were shorter than those in the control group (P<0.05); in addition,the total effective rate in the observation group was higher than that in the control group (P<0.05).Conclusion Compared with traditional operation,comprehensive therapy can improve the treatment efficiency,clinical symptoms and immune function of patients with early inflammatory intestinal obstruction,which deserves clinical promotion.

    Comparison of clinical effects and safety of different endoscopic biliary stent in the treatment of malignant obstruction

    Objective To compare the clinical effects and safety of different endoscopic biliary stent in the treatment of malignant obstruction.Methods A total of 90 malignant obstruction patients with endoscopic stent implantation from July 2011 to May 2018 were selected,and divided into group A and group B by using random number table,with 45 cases in each group.Patients in group A were given biliary plastic stent implantation,while patients in group B were given biliary metallic stent implantation.The success rate of stent implantation,indices of liver function and complications were compared between the two groups.Results There was no statistical difference in the success rate of stent implantation between the two groups (P>0.05).After 3 days of surgery,the levels of glutaminyl transferase (GGT),alkaline phosphatase (ALP),direct bilirubin (DBIL),total bilirubin (TBIL) in group B were lower than those in group A (P<0.05).The incidence rate of postoperative complications in group B was lower than that in group A (P<0.05).Conclusion Endoscopic metallic biliary stent implantation in the treatment of malignant obstruction can effectively improve patients’ liver function and decrease the incidence of complications,which is of safety.

    Minimally invasive aortic and mitral valve replacement or repair via partial upper hemisternotomy in 5 patients

    Objective To summarize the clinical experience of minimally invasive double valve replacement or repair via partial upper hemisternotomy.Methods Five patients with combined aortic valve and mitral valve disease underwent aortic replacement,mitral valve replacement,or repair via partial upper hemisternotomy with cardiopulmonary bypass through femoral artery,femoral vein,and superior vena cava cannulation,under general anesthesia with single lumen endotracheal intubation.Results There were no death cases,prolonged incision,second thoracotomy hemostasis,or tracheal intubation.The operation time was longer than routine operation; cardiopulmonary bypass time,aortic clump time,postoperative mechanical ventilation time,drainage,blood transfusion volume,and ICU time by designed operation were similar to those by routine operation.At 3-month,6-month,and 1-year follow-ups,the valves were with good function and there was no obvious proliferation of incision scar.Conclusion It is safe and feasible to perform double valve surgery via partial upper hemisternotomy.Compared with traditional operation,it can obtain similar therapeutic effect,and the short-term follow-up effect is reliable.

    Application of 3D printing technique to complex proximal humerus fractures

    Objective To reconstruct complex fracture models using 3D printing technique and explore the role of 3D printing technique in treating proximal humeral fractures.Methods Fifty patients with complex proximal humeral fractures admitted from June 2015 to June 2017 were enrolled in this study,and were randomly divided into two groups.The patients in group A received conventional treatment,while the patients in group B received adjuvant therapy that complex fractures were reconstructed using 3D printing techniques in addition to conventional treatment.The clinical indicators of surgery duration,blood loss volume during surgery,fracture healing time,and the number of C-arm fluoroscopy during surgery were compared between the two groups.The Neer Shoulder Scale (Neer score) and the CMS shoulder scoring system (CMS score) were used before and 12 months after the surgery to evaluate the shoulder function of the patients.Results After the surgery,the clinical indicators of group B were better than those of group A (P<0.05).The Neer score and CMS score of the two groups were both improved after 12 months of follow-up.The scores of the patients in group B were better than those in group A,and the differences were statistically significant (P<0.05).Conclusion 3D printing technique makes it possible to perform simulation surgery before real surgery,and present surgical procedures to physicians in advance and more intuitively.Applying the technique to the treatment of complex proximal humeral fractures can not only reduce operation time,bleeding volume during operation,and healing time,but also restore shoulder joint functions more effectively,greatly improving the quality of surgery,which deserves further applicaiton to fracture repair.

    Effect of transurethral bipolar plasmakinetic resection of prostate on patients with benign prostatic hyperplasia

    Objective To explore the effect of transurethral bipolar plasmakinetic resection of prostate (TUPKRP) on patients with benign prostatic hyperplasia.Methods A group of 50 patients with benign prostatic hyperplasia treated with TUPKRP from January 2017 to April 2018 were selected as observation group,and another group of 50 patients with benign prostatic hyperplasia treated with transurethral resection of prostate (TURP) at same period were selected as control group.The therapeutic effects were compared between two groups.Results The difference in total effective rate between the two groups was not statistically significant (P>0.05),nor were the differences in the time of washing bladder,removed gland weight,IPSS,RU,and Qmax (P>0.05).The surgery time,blood loss volume during surgery,QoL,IIEF-5 score,and complication rate in the observation group were significantly better than those in the control group (P<0.05).Conclusion TUPKRP applied to benign prostatic hyperplasia could reduce intraoperatve blood loss,reduce the incidence of complications,ensure curative effect,improve the quality of life and sexual function of patients,and therefore it is worthy of clinical application.

    Effect of ureteroscopy combined with holmium laser lithotripsy on upper ureteral calculi

    Objective To investigate the effect of ureteroscopy combined with holmium laser lithotripsy on upper ureteral calculi.Methods Ninety-two patients with upper ureteral calculi treated in our hospital were divided into contrel group and study group.Patients with even-numbered ureteral calculi were treated as study group (n=46) and those with odd number as reference group (n=46).The study group was treated with ureteroscopy combined with holmium laser lithotripsy,and the control group with laparoscopic ureterotomy.Complications,analgesics usage,operation and hospital stay were observed and compared between the two groups.Results After operation,compared with the control group,the study group had significantly shorter hospital stay and operation time,as well as significantly lower rates of complications and analgesics usage (P<0.05).Conclusion In the treatment of upper ureteral calculi,ureteroscopy combined with holmium laser lithotripsy is superior to laparoscopic ureterotomy in the treatment of upper ureteral calculi for its short operation time,minimal invasion,and high clinical value.

    Comparison of astigmatism after femtosecond laser surgery and phacoemulsification in patients with cataract

    Objective To compare the surgically induced astigmatism after femtosecond laser surgery and phacoemulsification in patients with cataract.Methods A total of 100 patients with cataract (100 eyes) were divided into an experimental group (n=50) and a control group (n=50).The experimental group received femtosecond laser surgery,while the control group received traditional phacoemulsification surgery with clear corneal incision at 10 the o’clock position.Uncorrected visual acuity (UCVA),best corrected visual acuity (BCVA),astigmatism,corneal thickness,and surgically induced astigmatism (SIA) were compared between the two groups before and after surgery.Results The UCVA in the two groups 1 day,1 week,1 month,and 3 months after surgery was improved compared with the indicator before surgery (P<0.05),but had no statistical differences between the two groups (P>0.05).The BCVA in the two groups were not statistically different 1 day,1 week,and 3 months after surgery (P>0.05),but 1 month after surgery,the BCVA in the control group was better than that in the experimental group (P<0.05).The astigmatism,corneal thickness,and SIA before surgery and 3 months after surgery were not different between the two groups (P>0.05).Conclusion Although both femtosecond laser surgery and phacoemulsification result in better UCVA at the early period after surgery,but the former effect is faster.

    Clinical comparison between endoscopic thyroidectomy via breast areola approach and open surgery

    Objective To compare the clinical effects of endoscopic thyroidectomy via breast areola approach and open surgery.Methods A total of 96 female patients with benign thyroid tumor treated from June 2017 to June 2018 in Heping County People’s Hospital were selected and assigned to two groups by random numbertable,with 48 cases in each group.The control group received open surgery; the observation group received endoscopic thyroidectomy via breast areola approach.The operation conditions,incidence rate of postoperative complications,and postoperative cosmetic satisfaction were compared.Results The intraoperative blood loss volume,postoperative drainage volume,operation time,and hospitalization time of the observation group were less than those of the control group,while the cosmetic satisfaction rate of the observation group (91.67%) was higher than that of control group (75.00%) (P<0.05).The incidence rate of postoperative complications between the two groups was not significantly different (χ2=0.716,P>0.05).Conclusion Applying endoscopic thyroidectomy via breast areola approach can reduce intraoperative blood loss volume and postoperative drainage volume,shorten operation time and hospitalization time,and improve postoperative aesthetic satisfaction.

    Objective To enhance awareness and attention through demonstrating clinical characteristics of Listeria monocytogenes infection during pregnancy as well as its treatment and prognoses.Methods A retrospective study of two listeriosis cases who were treated in Tongji Hospital of Tongji University was conducted.Patients’ main complains,clinical features,laboratory examinations,severity of the disease,treatment,and maternal and infant outcomes were studied.The clinical characteristics of listeriosis in perinatal pregnant women were discussed based on relevant literature.Results These 2 mothers were diagnosed with Listeria monocytogenes infection combined with bacteremia.Both cases ended in bacteremia and intrauterine fetal death.Conclusion Listeriosis easily infects pregnant women with relatively low immunity and causes intrauterine infection through vertical transmission,resulting in adverse pregnancy outcomes.We should improve practioners’ vigilance for pregnant women infected with Listeria monocytogenes so as to achieve early detection,early diagnosis,and early treatment,and improve perinatal survival.

    Analysis on influence of current doctor-patient relationship on career choices of medical students and young doctors

    Objective To explore the influence of current doctor-patient relationship on the profession choices of medical students and young doctors,analyze the reasons why they are unwilling to become doctors,and give suggestions to solve the problem.Methods Medical students,young doctors,and patients were asked to complete a questionnaire survey(n=1500).Data on recognition of doctor-patient relationship,medical students’ career concerns,career development,and mental health,were collected.Results About 29.24% of medical students and 50.30% of young doctors believed that the doctor-patient relationship was “very intense”; 66.26% of medical students and 70.99% of young doctors considered doctor a high-risk job.Medical violence brought more psychological injury to doctors.Conclusion The intense doctor-patient relationship has seriously affected the mental health of medical students and young doctors,and their career choices.The main reason for not wanting to be a doctor is the negative doctor-patient relationship.To reduce this adverse impact,we need to pay attention to the cultivation of humanistic quality,legal consciousness,and communication skills for medical students and young doctors.

    Practice of procurement process optimization based on ventilator performance

    Objective To explore the optimization of ventilator procurement process based on the analysis of ventilator performance.Methods Through the analysis of the evaluation method for ventilator procurement,the operating environment,hardware configuration,performance parameters,physical factors,and performance test of ventilators were discussed.Results A centralized management approach on procurement mode and maintenance of ventilators were summarized.Conclusion Ventilator procurement process optimization based on ventilator performance is beneficial to procurement decision-makings,which is of long-term significance for the clinical application and efficiency of ventilators.

    Study on multi-sector joint supervision in reducing rust incidence after medical instrument disinfection by automatic washing machines

    Objective To explore the improvement measures of joint supervision by multiple sectors for reducing the incidence of rust of apparatus after disinfection by automatic washing machines.Method Multiple sectors worked together to obtain Pareto diagram of rust locations through statistics,and found out the key links and key factors that affected the rust after disinfection by automatic washing machines.Countermeasures were made and implemented according to the main causes of rust,and then the indicators before and after the implementation were evaluated.Results Six improvement measures were formulated and implemented.After improvement,the rust incidence of medical instrument after disinfection by automatic washing machines was reduced from 34.7% to 12.9%.The target achievement rate was 110.66%,and the improvement rate was 62.82%,confirming that the improvement work was effective.Conclusion Multi-sector joint supervision allows a more detailed cause analysis,more feasible improvement measures,and a more notable result.

    Curative effect of ankle pump on ankle joint swelling after operation

    Objective To observe the effect of the multi-mode detumescence scheme of antiswelling drug combined with ice compress,elevation of affected limb,and ankle pump in the perioperative period of ankle fracture surgery.Methods A total of 40 cases of ankle fractures treated in the authors’ department were included between January 2015 and June 2017,and randomly divided into experimental group and control group by the table of random numbers.The experimental group was treated with intravenous injection of sodium aescinate at 20 mg daily,postoperative ankle ice twice a day,affected limb raising,and ankle pump nursing.The control group received the same treatment except ankle pump.Visual analogue scale (VAS) score and ankle circumference were observed 6 h and 72 h after surgery.Results There was no significant difference between the experimental group and the control group in VAS score and ankle circumference at postoperative 6 h.The VAS scores and the ankle circumferences of the experimental group and the control group at postoperative 72 h were statistically lower than those at postoperative 6 h (P<0.05),and the two indicators of the experimental group at postoperative 72 h were statistically lower than those of the control group (P<0.05).Conclusion Applying ankle pump can effectively relieve postoperative pain and swelling of patients,which is beneficial to their recovery.

    Observation of endoscopic hemostasis treatment and nursing for acute non-varicose upper gastrointestinal bleeding

    Objective To investigate the effect of endoscopic hemostasis on acute non-varicose upper gastrointestinal bleeding and its symptomatic nursing.Methods Based on the etiology and endoscopic improved Forrest classification,173 cases of acute non-varicose upper gastrointestinal bleeding received different treatments,including injection of drugs,argon ion coagulation (APC),vascular clamp,and combined hemostatic therapy.Their curative effects and symptomatic nursing were observed.Results In the 173 patients with non-varicose upper gastrointestinal bleeding,the total immediate hemostasis rate was 98.27% (170/173),and the rebleeding rate was 8.09% (14/173).Among them,69 patients in the injection group had the immediate hemostasis rate of 92.75% (64/69),effective hemostasis rate of 85.51% (59/69),rebleeding rate of 11.59%(8/69),and surgical operation rate of 10.14% (7/69); 8 cases in the APC group had the immediate hemostasis rate of 87.50% (7/8),effective hemostasis rate of 75.00% (6/8),and rebleeding rate of 25.00%(2/8); 91 patients in the vascular clamp group had the immediate hemostasis rate of 100% (91/91),effective hemostasis rate of 96.70% (88/91),rebleeding rate of 3.30% (3/91),which was significantly lower than those of the injection group and the APC group (P<0.05),and surgical operation rate of 2.2% (2/91); 5 patients in the combined group showed the immediate hemostasis rate of 100% (5/5),effective hemostasis rate of 80.00%(4/5),rebleeding rate of 20.00%(1/5),and surgical operation rate of 20.00%(1/5).Neither significant complications nor deaths occurred.Conclusion Endoscopic hemostasis exhibits a quick and precise effect; therefore,it should be the preferred first-aid treatment for acute non-varicose upper gastrointestinal bleeding patients.The key to successful rescue of the disease includes accurate assessment,fast recovery of effective circulation,endoscopic hemostatic treatment appropriate for bleeding etiologies,and close cooperation between doctors and nurses.

    Qualitative study on marriage experience and treatment of homosexual male patients with AIDS

    Objective To understand the real life experience of HIV/AIDS infected homosexual males in marriage relationship and receiving anti-viral treatment,in order to provide guidance for better case management.Methods Semi-structured interviews were conducted among 10 married AIDS homosexual male patients,and the data were analyzed using Colaizzi phenomenological analysis.Results Finally 5 topics including “shame and self-discrimination”,“worrying about wife and children being infected and discriminated”,“the pressure and hope of marriage for treatment”,“not knowing how to inform disease status to spouse and children”,and “sense of uncertainty towards prognosis”.Conclusion The marriage between a gay and a female brings psychological pressure to AIDS infected gay patients,as well as impacts on their treatment.This group has ethical dilemmas,and their lifestyle affects the spread of diseases.Therefore,trapeted intervention provided by professional medical staff are required.

    Clinical practice of reducing hospitalized puerperas’ breast distending pain by applying quality control circle activity

    Objective To explore the effect of quality control circle on reducing the incidence of hospitalized puerperas’ breast distending pain.Methods A total of 208 hospitalized puerperas who delivered from March to October in 2017 were selected as study subjects.After setting up the quality control cycle group,we conducted cross-sectional investigations,performed cause analysis,made plans,and formulated countermeasures to intervene in the factors of puerperas’ breast distending pain after delivery.Results The incidence rates of hospitalized puerperas’ breast distending pain decreased from 33.3% before the activity to 15.09% after the activity.Besides,the puerperas’ breast pain and swelling were improved obviously after the quality control circle activity (P<0.05).Conclusion The quality control circle can not only reduce the incidence of hospitalized puerperas’ breast distending pain,but also improve nursing quality.
