    The influence of bufalin inhibits TGF-β1-induced epithelial mesenchymal transition in human colon cancer HCT-116 cells

    Objective In order to explore the possible mechanism between bufalin and colorectal cancer, we have observed the effects that bufalin inhibits the epithelial mesenchymal transformation on colorectal cell HCT-116. Methods We induce the colorectal cell HCT-116 to take effect of epithelial-mesenchymal transition (EMT) by TGF-β1 to prepare the EMT models. The experiment is divided into control groups, model groups and bufalin groups. After 72h of intervention, observe the morphological difference of each groups; evaluate the effect of the invasion and migration capabilities of bufalin on HCT-116 by using transwell-invasion and transwell-migration models; To detect the expressions of EMT-related protein including E-cadherin,Vimentin and β-catenin by using Western blot; To observe the distribution of β-catenin protein by using immunofluorescence method. Results 1. Changes of cellular morphology: Compared with the control groups, the cells of the model groups became elongated and spindle, the connections between cells are relatively lax. 2. Transwell-invasion experiment shows: After 72h , the cell numbers of permeation(under ×200, 3 view) of control groups, model groups and bufalin groups are: (86.67±11.72), (413.33±41.63) and (110.00±26.46), which show significant difference(P<0.05). Transwell-migration experiment shows: After 72h , the cell numbers of permeation(under ×200, 3 view) of control groups, model groups and bufalin groups are: (475±69.41), (898±49.49) and (507±38.16), which show significant difference(P<0.05). 3. The expressions of EMT-related protein E-cadherin/Vimentin/β-catenin by Western blot: Compared with the control groups, the model groups have lower expressions of E-cadherin and higher expressions of Vimentin; compared with model groups, the Bufalin groups have higher expressions of E-cadherin while the expressions of Vimentin are significantly lower. There is no significant difference between the expressions of β-catenin. 4. The distribution of β-catenin under immunofluorescence method: The experiment shows, compared with the control groups, the model groups have β-catenin expressions moving from the cell membrane and the cytoplasm to nucleus; compared with the model groups, the Bufalin groups have more expressions in cytoplasm than in nucleus. Conclusion 1. TGF-β1 can induce the epithelial mesenchymal transformation of colorectal cell HCT-116. 2. The inhibiting ability of Bufalin to the invasion and migration of HCT-116 is related to the epithelial mesenchymal transformation. 3. Bufalin probably inhibits the EMT of HCT-116 by inhibiting the nuclear translocation of β-catenin, and thus inhibit the invasion and migration of cells.

    Evaluataion of endoscopic ultrasound guided fine needle aspiration(EUS-FNA) in diagnosis of occupying lesions in upper digestive tract

    Objective To evaluate the accuracy and clinical application of endoscopic ultrasound guided fine needle aspiration(EUS-FNA) in diagnosis of occupying lesions in upper digestive tract and its peripheral areas.Methods Forty-six patients underwent endoscopic ultrasonagraphy and EUS-FNA between November 2011 and December 2013.The result of cytology and/or pathology was compared with that of surgical finding and follow-up study.Results EUS-FNA was successfully performed on 35patients,with a success rate of100%. The35patients with lesions in oesophagus (4 cases), stomach(26 cases), duodenalbulb(5 cases). Among them,the lesions in 29 patients (82.85%) were cytologically confirme.The lesions in10 out of15 patients (60.66%) were pathologically confirmed.Compared with the results of pathology and long-term followe-up study,the overall diagnostic accuracy of EUS-FNA in diagnosis of lesions in upper digestive tract and its peripheral areas was 85.71%(30/35). There were no complications during the procedures. Conclusion EUS-FNA is a safe, effective and accurate method for diagnosis of occupying lesions in upper digestive tract and its peripheral areas.

    Observing the Application Effect of Endoscopic Mask and Preoxygen on Painless Gastroscopy in Elderly Patients

    Abstract objective: to evaluate the application effect of endoscopic mask and pre-oxygen on the painless gastroscopy in elderly patients. Methods: 100 elderly patients with painless gastroscopy were randomly divided into two groups: In A group, patients were with 3L/min nasal catheter oxygen. In B group, patients were with 3L/min endoscopic mask oxygen and 2L/min pre-oxygen. Two groups were anesthetized with intravenous injection of propofol. Pulse oxygen saturation (SpO2) lower than 90% was as respiratory depression. Mean arterial pressure (MAP), heart rate (HR), and SpO2 were recorded in the two groups before and after the examination. The operation time and adverse reaction were also recorded. Results: The MAP in two groups and SpO2 in nasal catheter oxygen group significantly decreased during operation (P<0.05). During operation, there were significant differences in SpO2 between two groups. The SpO2 in A group was significantly lower than that in B groups. In addition, the operation time and the rate of adverse reactions were higher in A group than those in B group (P=0.001<0.05). Conclusion: the use of endoscope and the application of pre-oxygen inhalation for elderly patients with painless gastroscopy can significantly increase the oxygen content in elderly patients, increase the oxygen reserves and the body's tolerance to hypoxia. Conclusion: Application of endoscopic mask and pre-oxygen in elderly patients with painless gastroscopy can significantly increase the oxygen content of elderly patients, increase the oxygen reserves and the body's tolerance to hypoxia, stabilize the functions of circulation and respiratory and have better security

    Clinical effect of capecitabine combined with oxaliplatin in treating elderly patients with gastric cancer

    Abstract Objective: To investigate the clinical effect of capecitabine combined with oxaliplatin in treating elderly patients with gastric cancer. Methods: 120 elderly patients with gastric cancer in our hospital from October 2014 to October 2015 were selected. They were divided into control group (n=60, given 5-fluorouracil combined with oxaliplatin) and observation group (n=60, given capecitabine combined with oxaliplatin). Results: Total efficiency of observation group (n=54, 90.00%) was obviously higher than control group (n=39,65.00%). Compared the abnormal liver function, nausea and vomiting, leukopenia, abnormal sensory nerve, thrombocytopenia and buccal cell of the two groups. Incidence of complications of observation group was obviously lower than control group, the difference was statistically significant (P<0.05). Conclusion: Capecitabine combined with oxaliplatin in treating elderly patients with gastric cancer has better clinical effect, the incidence of complications is lower and is worthy of clinic application.

    Primary Radical Operation in the Treatment of Periamal Abscess-Clinical Observation

    [Abstract] Objective To evaluate the clinical effect between primary radical operation and incision and drainage of the treatment on crissum abscess.Methods 84 cases of crissum abscess were randomly divided into treatment group (n=44) and control group (n=40). Patients in treatment group were received primary radical operation, patients in control group were only treated by incision and drainage. Results Compared with the control group, treatment group patients postoperative abscess recurrence rate forming the incidence of anal fistula, wound healing time significantly lower than the control group obviously(p<0.001).Two groups were anal incontinence, anal stenosis and postoperative anal malformation, etc., were followed up for 1 year, anus function are normal.Conclusions It is suggested that primary radical operation can be primarily performed,lower the abscess recurrence and anal fistula incidence,which not only shorten treatment course,but also reduce pain of patients.Primary radical operation is a good method of crissum abscess therapy.

    Fracture dislocation at thoracolumbar junction treated with short-segment pedicle instrument

    [Abstract] Objective To study the efficacy of thoracolumbar spinal fracture-dislocation treated with posterior short-segment pedicle instrument. Methods Under general anaesthesia, 25 cases were treated by posterior route reduction and laminectomy and short-segment pedicle screw internal fixation and bone grafting. To appraise the efficacy through comparing the anterior and posterior height of fractured vertebrae, the Cobb's angle, the cross-section area of spinal canal, the spinal alinement and the nerve function between preoperative and postoperative. Result All cases were followed up. Length of follow-up ranged from 8to 48months. Spinal alinement was reduced normally. The average anterior and posterior height of fractured vertebrae was 31.2%、76.8% preoperatively, 83.5%、92.6%postoperatively; Cobb's angle was 32.5°preoperatively, 7.6°postoperatively. The cross-section area of spinal canal was 36.5% preoperatively, 86.4% postoperatively. There was significant deference between pre-operation and post-operation(P<0.01). Nerve recovery: 2 cases of complete paraplegia had no recovery, others recovered 1~3 grades by Frankel.1 case of broken pedicle screws occurred. Conclusion Short-segment pedicle instrument is suitable for treating thoracolumbar spinal fracture-dislocation, which has advantages of simplicity, safety and less segment fixed, reduced spinal alinement.

    Clinical Research of Tension-Free Herniorrhaphy in the Treatment of Inguinal Incarcerated Hernia

    Objective:To observe the tension-free herniorrhaphy in the treatment of inguinal incarcerated hernia. Method:100 patients with inguinal incarcerated hernia treated from April 2013 to March 2014 in our hospital were selected and randomly divided into study group and control group, 50 cases each group. The study group adopted the tension-free herniorrhaphy; the control group adopted the conventional herniorrhaphy. The clinical index like medical cost, hospitalization time, time of leaving bed, operation time as well as incidence of postoperative complications of two groups was compared. Result: The clinical index like medical cost, hospitalization time, time of leaving bed and operation time of study group was significantly better than that of control group (P<0.05); the incidence rate of complications like low-grade fever, hydrocele of testes, incision infection, uroschesis of study group was significantly lower than that of control group (P<0.05); Conclusion:The tension-free herniorrhaphy is easy to operate, has an obvious effect in the treatment of inguinal incarcerated hernia and has fewer postoperative complications and fast recovery. It is worthy of clinical promotion.

    The effectiveness of different anaesthetics for Motor Evoked Potentials during cervical spine surgery

The MEP monitoring need higher requirement for operating skills and environment, and is often effected by anesthetics, particularly the muscle relaxant.     Anesthetic management of huge substernal goiter with a novel endoscopic nasal mask: a case report

    Objective To elevate the safety of a airway obstructed patient caused by huge substernal goiter in anesthesia induction and tracheal intubation with a novel endoscopic nasal mask. Methods A novel endoscopic nasal mask was used in the process of awake oral fiberoptic bronchoscopy guided tracheal intubation. Results The first attempt of tracheal intubation was successful under the guide of bronchoscope without oxygen saturation decrease, cough and other adverse reactions. Conlcusion The novel endoscopic nasal mask could enhance patient safety in difficult airway intubation and with great clinical application value.

    Rosai-Dorfman disease one cases report and literature review

    【Abstract】 Object :To investigate the clinical and pathological featu-res, diagnosis, differential diagnosis and treatment of Rosai-Dorfman dis-ease. Methods:One cases of Rosai-Dorfman disease of skin were exa-mined by tissue morphology ,immunohistochemistry and special stainin-g ,and the related liter-atures were followed up and reviewed. Results :Skin tissue in the der-mis see more lymph and plasma cells and tissue ce-ll hyperplasia, flaky, meantime visible multinucleated giant cells (S-100 +), and phagocytosis of lymphocytes. Immunohistochemistry: CD3 (+ +), CD20 (+ +), CD79a (+ +), CD68 (+), S-100 (+), CD138 (+). Conclusion: Rosai Dorfman disease is a rare, unexplained idiopathic organization cell hyperplasia disease, it need to differential diagnosis with Langerhans cell histiocytosis,granulo-matous infection, tissue cell phagocytosis pannicul-itis,xanthogranuloma and so on. The diagnosis mainly depends on the m-orphology and immunohistochemical markers. There are many treatme-nt methods, the current views have not been unified.

    Different Methods of Alimentary Tract Reconstruction After Gastrectomy

    Stomach cancer is one of the most common malignant neoplasm diseases. After gastrectomy for gastric cancer, the integrity of the alimentary tract was destroyed, influencing the functions of stomach such as storage, mixture of food, initial digestion and absorption of many substances, which finally leads to malnutrition and poor quality of life. Therefore, the selection of the methods of reconstruction is particularly significant. The best reconstruction method is one that keeps patients maintain satisfactory nutritional status and quality of life and keeps postoperative morbidity as low as possible. This review focuses on the different methods of alimentary tract reconstruction.

Sino system for coronary operation risk evaluation(SinoSCORE) is the first cardiac surgery risk evaluation system in which is widely used in clinical practice.     Advances in corneal intrasteomal injection for fungal keratitis

    As a serious blinding eye disease, fungal keratitis (FK), often tends to invade into deep corneal stroma. Clinically, the main antifungal treatments are through the oral and intravenous systemic administrations, combined with locally used antifungal eyedrops. But these treatment pathways are often has a poor effects for some intractable FK patients. Recently, directly intraconeal stroma injection of antifungal drugs has been revealed having a good clinical efficacy, even for the intractable cases. In this paper, this new treating method was reviewed.

    Analysis of related factors of prenatal anxiety on choice of delivery pattern

    Abstract:Objective Investigate prenatal anxiety and childbirth way choice.Analyze the relations between prenatal anxiety and the choice of delivery mode.Methods Choosing tongji hospital in July 2013 to December 2013 obstetrics outpatient middle-late 210 cases of pregnant women, and adopts the questionnaire of basic situation and self-evaluation of anxiety scale (SAS) .Pregnant women can be divided into observation and control group, compared two groups of pregnant women in the cesarean delivery rate, and study the influencing factors of pregnant women anxiety level.Results The women surveyed the incidence of prenatal anxiety was 15.5%.Observation group cesarean delivery rate was 70.6%. Control group cesarean delivery rate was 57.9%. The observation group is significantly higher than without anxiety.At the age of 35 is bounded, the anxiety level is higher than non-pregnant women pregnant, statistically significant difference (P < 0.01).Before conception and during pregnancy on maternal health anxiety level effect have obvious difference (P < 0.01).Pregnant women with different education, occupation, monthly income has no obvious difference in prenatal mood.Conclusion The elderly, pre pregnancy health status and health status during pregnancy can lead to anxiety of pregnant women.The choice of the ways of prenatal anxiety childbirth for the pregnant woman has a certain influence.
